舒芬太尼经自控镇痛泵(PCA)治疗难治性癌痛的疗效分析

Analysis of Curative Effect of Sufentanil via PCA in the Treatment of Refractory Cancer Pain

目的探讨舒芬太尼经自控镇痛泵(PCA)在难治性癌痛患者中的疗效。方法便利选择2018年1月—2019年10月在该院住院的难治性癌痛患者50例,随机分为观察组和对照组,每组25例。观察组患者小剂量持续静脉给药以自控镇痛泵进行镇痛治疗,对照组患者口服羟考酮缓释片治疗癌痛。比较两组患者治疗前、治疗后2 h、12 h、24 h、48 h和72 h的NRS评分,以及治疗中并发症的发生率。结果两组患者治疗后均表现疼痛程度的降低(5.7±1.8)分vs(3.1±0.8)分,(5.4±1.2)分vs(2.4±0.6)分,差异有统计学意义(P<0.05)。重复测量的方差分析显示观察组患者在治疗过程中的疼痛程度显著低于对照组,差异有统计学意义(F=45.230,P<0.001)。对照组患者总并发症发生率显著高于观察组,差异有统计学意义(χ^(2)=11.538,P=0.001)。结论舒芬太尼经PCA治疗难治性癌痛患者具有控制疼痛效果好,并发症发生率低等特点。

Objective To investigate the efficacy of sufentanil via a self-controlled analgesic pump(PCA)in patients with refractory cancer pain.Methods 50 patients with refractory cancer pain who were hospitalized in the hospital from January 2018 to October 2019 were conveniently selected and randomly divided into observation group and control group,25 cases in each group.Patients in the observation group were given low-dose continuous intravenous administration with a self-controlled analgesic pump treatment,patients in the control group took oral oxycodone sustained-release tablets for cancer pain.The NRS scores of the two groups of patients before treatment,2 h,12 h,24 h,48 h and 72 h after treatment,and the incidence of complications during treatment were compared.Results Both groups of patients showed a decrease in pain degree after treatment(5.7±1.8)points vs(3.1±0.8)points,(5.4±1.2)points vs(2.4±0.6)points,and the difference was statistically significant(P<0.05).Repeated measures analysis of variance showed that the pain degree of the observation group was significantly lower than that of the control group,and the difference was statistically significant(F=45.230,P<0.001).The incidence of total complications in the control group were significantly higher than that in the observation group,and the difference was statistically significant(χ^(2)=11.538,P=0.001).Conclusion PCA treatment of sufentanil in patients with refractory cancer pain has the characteristics of good pain control and low complication rate.
