集束化护理在降低非计划性拔管中的应用

Application of Cluster Nursing in Reducing Unplanned Extubation

目的探究集束化护理在降低非计划性拔管中的应用效果。方法方便选择2018年1月—2019年7月该院ICU患者70例,随机进行分组,每组35例,一组为对照组,予实施常规护理,另外一组为观察组,予实施集束化护理,然后收集两组患者非计划性拔管发生例数、住院时间、临床疗效等数据并进行分析。结果与对照组相比,观察组住院时间(14.92±11.20)d和转出ICU时间(11.45±14.01)d均更短,差异有统计学意义(t=2.057、2.137,P<0.05);观察组患者治疗总有效率91.43%显著更高,差异有统计学意义(χ^(2)=4.629,P<0.05);观察组患者非计划性拔管2.86%更低,差异有统计学意义(χ^(2)=5.081,P<0.05);观察组患者在护理态度(89.53±2.08)分,护患沟通(91.24±1.90)分,护理执行力(90.76±1.97)分,健康宣讲(90.83±1.82)分方面均更高,差异有统计学意义(t=34.792、41.802、34.489、32.312,P<0.05)。结论对于ICU患者,集束化护理的效果是非常显著的,可在很大程度上降低非计划性拔管发生率,提高患者对护理服务的满意程度,值得推荐。

Objective To explore the effect of cluster nursing in reducing unplanned extubation.Methods From January 2018 to July 2019,70 ICU patients in the hospital were convenient selected and randomly divided into groups.Each group had 35 cases.One group was the control group and was given routine care,and the other group was the observation group with clustered nursing,and then collect and analyze data on the number of cases of unplanned extubation,length of stay,and clinical efficacy of the two groups of patients.Results Compared with the control group,the observation group's hospitalization time(14.92±11.20)d and the transfer time(11.45±14.01)d were shorter,the difference was statistically significant(t=2.057,2.137,P<0.05);the observation group always had treatment efficiency of 91.43%was significantly higher,the difference was statistically significant(χ^(2)=4.629,P<0.05);the unplanned extubation of patients in the observation group was 2.86%lower,the difference was statistically significant(χ^(2)=5.081,P<0.05);the patients in the observation group were in nursing attitude(89.53±2.08)points,nurse-patient communication(91.24±1.90)points,nursing executive power(90.76±1.97)points,health promotion(90.83±1.82)points were all higher,the difference was statistically significant(t=34.792,41.802,34.489,32.312,P<0.05).Conclusion For ICU patients,the effect of clustered care is very significant,which can reduce the incidence of unplanned extubation to a large extent and improve patients'satisfaction with nursing services.It is worth recommending.
