重症妊娠高血压综合征患者剖宫产术的麻醉处理

Anesthesia Management of Cesarean Section in Patients with Severe Pregnancy-induced Hypertension

目的探究分析重症妊娠高血压综合征患者剖宫产术的麻醉处理方法及效果。方法2017年7月—2019年7月该院共收治102例重度妊娠高血压综合征行剖宫产产妇,按照不同的麻醉处理方式将产妇分为A组(腰硬联合麻醉)、B组(硬膜外麻醉)、C组(全身麻醉),比较3种麻醉处理的效果。结果A组麻醉效果优良率33例(97.06%)相比于B组30例(88.24%)、C组26例(76.47%)较优,组间差异有统计学意义(P<0.05),A组麻醉诱导时间(4.78±3.02)min、感觉阻滞起效时间(8.82±1.22)min、平均住院时间(11.1±3.4)d相比于B组(12.18±4.87)min、(9.98±1.11)min、(13.2±3.5)d、C组(14.16±5.21)min、(1.27±11.89)min、(14.1±3.2)d较短,3组数据对比差异有统计学意义(P<0.05)。A组不良反应发生率11.76%与B组不良反应发生率14.70%、C组不良反应发生率11.76%比较,差异无统计学意义(P>0.05)。结论合理的麻醉方法可促使剖宫产顺利进行,可促使患者快速进入麻醉状态,可缩短患者的住院时间。

Objective To explore and analyze the anesthesia treatment methods and effects of cesarean section in patients with severe pregnancy-induced hypertension.Methods From July 2017 to July 2019,the hospital admitted 102 cases of severe pregnancy-induced hypertension who underwent cesarean section.The women were divided into group A(combined spinal-epidural anesthesia)and group B(hard Extra-membrane anesthesia),group C(general anesthesia),compare the effects of three anesthesia treatments.Results The excellent and good rate of anesthesia effect in group A was 33 cases(97.06%)compared with 30 cases(88.24%)in group B and 26 cases(76.47%)in group C.The difference between groups was statistically significant(P<0.05),group A Anesthesia induction time(4.78±3.02)min,sensory block onset time(8.82±1.22)min,average hospital stay(11.1±3.4)d compared with group B(12.18±4.87)min,(9.98±1.11)min,(13.2±3.5)d、(14.16±5.21)min,(11.27±1.89)min,(14.1±3.2)d in group C were shorter,and the difference between the three groups was statistically significant(P<0.05).Group A:Compared with the adverse reaction rate of 11.76%in group B and 14.70%in group B and 11.76%in group C,the difference was not statistically significant(P>0.05).Conclusion A reasonable method of anesthesia can promote the smooth progress of cesarean section,prompt the patient to enter anesthesia quickly,and shorten the patient's hospital stay.
