四肢CTA在血管损伤诊断中的价值评价

摘要 目的评价在血管损伤诊断中应用四肢CTA的价值。方法选择该院2018年4月—2019年5月间收治的血管损伤患者74例作为分析对象,74例患者分别接受增强CT和四肢CTA检查,对比组间血管损伤患者增强CT和四肢CTA诊断结果、增强CT和四肢CTA诊断准确率。结果患者经增强CT和四肢CTA诊断检查后诊断结果经四肢CTA检查造影剂外溢(100.00%)、假性动脉瘤(56.76%)、动-静脉瘘(100.00%)与增强CT检查(98.65%)、(55.40%)、(97.30%),比较差异无统计学意义(χ^(2)_(造影剂外溢)=1.007,P=0.316;χ^(2)^(假性动脉瘤)=0.027,P=0.868;χ^(2)_(动-静脉瘘)=2.027,P=0.154)。组间患者经不同检查后诊断准确率经四肢CTA检查(98.65%)与增强CT检查(94.59%),比较差异无统计学意义(χ^(2)=1.863,P=0.172)。结论四肢CTA、增强CT检查血管损伤均能够清晰显示断裂或损伤部位,明确损伤周围有无血栓形成,为外伤患者明确是否发生血管损伤提供直接依据。 Objective To evaluate the value of applying extremity CTA in the diagnosis of vascular injury.Methods The 74 patients with vascular injury admitted to the hospital from April 2018 to May 2019 were selected as the analysis object.74 patients received enhanced CT and limb CTA examination,respectively.The diagnosis results of enhanced CT and limb CTA in patients with vascular injury between groups were compared.Enhance the diagnostic accuracy of CT and extremity CTA.Results After the patient undergoes enhanced CT and extremity CTA diagnostic examination,the diagnosis result was examined by extremity CTA examination of contrast agent spillage(100.00%),pseudoaneurysm(56.76%),arteriovenous fistula(100.00%)and enhanced CT examination(98.65%),(55.40%),(97.30%),were statistically tested,the difference was not statistically significant(χ^(2) _(contrast agent overflow)=1.007,P=0.316;χ^(2) _(pseudoaneurysm)=0.027,P=0.868;χ^(2)_(arteriovenous fistula)=2.027,P=0.154).After different examinations,the diagnostic accuracy of patients between the groups was statistically tested by limb CTA examination(98.65%)and enhanced CT examination(94.59%),and the difference was not statistically significant(χ^(2)=1.863,P=0.172).Conclusion Extremity CTA examination and enhanced CT of vascular injury can clearly show the location of the fracture or injury,and determine whether there is thrombosis around the injury,and provide a direct basis for trauma patients to determine whether vascular injury has occurred.
