经阴道联合腹部超声对早期异位妊娠的诊断价值分析

Analysis of the Diagnostic Value of Transvaginal Combined Abdominal Ultrasound for Early Ectopic Pregnancy

目的研究经阴道联合腹部超声对早期异位妊娠的诊断价值分析。方法回顾性选择该院2017年6月—2018年6月收治的100例疑似早期异位妊娠患者,对全部患者实施阴道超声检查、腹部超声检查及联合检查诊断,评估各检查方法对异位妊娠诊断的符合率,对胎芽及胎心、卵黄囊、附件区包块的检出情况;对比各诊断方法的阳性率、灵敏度及特异度;分析超声声像图检测特征。结果临床手术病理证实90例存在异位妊娠,与单一腹部超声、阴道超声检测比较,阴道联合腹部超声检测的符合率较高,差异有统计学意义(P<0.05);与单一腹部超声、阴道超声检测比较,阴道联合腹部超声检查检出患者附件区包块、宫内假孕囊、胚芽、胎心搏动的比例均较高,差异有统计学意义(P<0.05);阴道联合腹部超声检测的阳性率、灵敏度、特异度分别为90.00%、98.89%、80.00%,高于单一腹部超声、阴道超声检测,差异有统计学意义(P<0.05)。结论阴道联合腹部超声检查可作为诊断异位妊娠的重要方法,其可提高诊断的符合率、特异性及灵敏度,降低出现误诊、漏诊的现象。

Objective To study the diagnostic value of transvaginal combined with abdominal ultrasound for early ectopic pregnancy.Methods A retrospective selection of 100 patients with suspected early ectopic pregnancy admitted to the hospital from June 2017 to June 2018 were performed vaginal ultrasonography,abdominal ultrasonography and joint inspection to diagnose all patients,and the conformity of each inspection method to the diagnosis of ectopic pregnancy rate,the detection of fetal buds,fetal heart,yolk sac,and adnexal masses was evaluated;compare the positive rate,sensitivity and specificity of each diagnostic method;analyze the detection characteristics of ultrasound imaging.Results Clinical operation and pathology confirmed 90 cases of ectopic pregnancy.Compared with single abdominal ultrasound and vaginal ultrasound,the coincidence rate of vaginal combined abdominal ultrasound was higher,the difference was statistically significant(P<0.05);compared with single abdominal ultrasound and vaginal ultrasound,vaginal combined abdominal ultrasonography detected a higher proportion of patients with adnexal mass,intrauterine pseudo-pregnancy sac,embryo,and fetal heart beat,the difference was statistically significant(P<0.05);the positive rate,sensitivity,and specificity of vaginal combined abdominal ultrasound were 90.00%,98.89%,80.00%,higher than single abdominal ultrasound and vaginal,the difference was statistically significant(P<0.05).Conclusion The combination of vaginal and abdominal ultrasound can be used as an important method for the diagnosis of ectopic pregnancy.It can improve the coincidence rate,specificity and sensitivity of diagnosis,and reduce the phenomenon of misdiagnosis and missed diagnosis.
