重症急性胰腺炎并发糖尿病酮症酸中毒的急诊救治及预后分析

Emergency Treatment and Prognostic Analysis of Severe Acute Pancreatitis Complicated with Diabetic Ketoacidosis

目的研究重症急性胰腺炎并发糖尿病酮症酸中毒的急诊救治及预后情况。方法纳入2017年3月—2020年3月该院66例重症急性胰腺炎并发糖尿病酮症酸中毒患者为研究对象,随机分为对照组与实验组,对照组33例采取常规急诊救治,实验组33例在常规组基础上采用小剂量胰岛素急诊救治。比较两组总有效率、空腹血糖、餐后2 h血糖、血淀粉酶、甘油三酯水平。结果治疗后,实验组治疗有效率高于对照组,差异有统计学意义(P<0.05);实验组空腹血糖、餐后2 h血糖、血淀粉酶、甘油三酯水平均优于对照组,差异有统计学意义(P<0.05)。结论对重症急性胰腺炎并发糖尿病酮症酸中毒采用小剂量胰岛素急诊救治,能有效提高治疗效果,改善各项功能指标。

Objective To study the emergency treatment and prognosis of severe acute pancreatitis complicated by diabetic ketoacidosis.Methods From March 2017 to March 2020,66 patients with severe acute pancreatitis complicated with diabetic ketoacidosis in the hospital were included as the research objects.They were randomly divided into the control group and the experimental group.The control group 33 cases received conventional emergency treatment,the experimental group 33 cases were treated with low-dose insulin for emergency treatment on the basis of the conventional group.The total effective rate,fasting blood glucose,blood glucose,blood amylase,and triglyceride levels 2 hours after a meal were compared between the two groups.Results After treatment,the effective rate of treatment in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05);the fasting blood glucose,blood glucose,blood amylase,and triglyceride levels of the experimental group were better than those in the control group after 2 hours after meal,the difference was statistically significant(P<0.05).Conclusion The emergency treatment of severe acute pancreatitis complicated by diabetic ketoacidosis with low-dose insulin can effectively improve the treatment effect and improve various functional indicators,which has clinical promotion significance.
