血清肿瘤标志物联合血清CA50、AFP预测结直肠癌肝转移的临床价值

Clinical Value of Detection of Serum Tumor Markers Combined with CA50,and AFP in Prognosticating Colorectal Cancer Liver Metastasis

为分析血清肿瘤标志物(CEA、β2-MG)联合血清CA50、AFP预测结直肠癌肝转移的临床价值,将80例结直肠癌患者根据术后病理结果将其分成对照组和观察组,其中对照组50例为未发生肝转移的患者,观察组30例为发生肝转移的患者。抽取患者肘静脉血10mL,离心处理后留下血清,采用全自动电化学发光仪及全自动酶分析仪检测血清CA50、AFP、CEA、β2-MG水平。比较2组患者CA50、AFP、CEA、β2-MG水平及各项指标阳性情况,计算单指标及联合指标的诊断价值,分析其特异度、灵敏度、准确度及尤登系数。结果显示,观察组患者CA50、AFP、CEA、β2-MG水平高于对照组(P<0.05);观察组患者CA50、AFP、CEA、四项联检阳性率高于对照组(P<0.05),β2-MG阳性率高于对照组,但比较差异无统计学意义(P>0.05)。四项联检的准确度和尤登系数最高。结果表明,血清肿瘤标志物CEA、β2-MG、CA50、AFP可作为结直肠癌及肝转移的诊断指标,联合检测有利于提升结直肠癌肝转移诊断的性能。

This study was to evaluate the clinical value of detection of serum tumor markers combined with CA50 and AFP in prognosticating colorectal cancer liver metastasis,according to their postoperative pathological exam.results divided 80 colorectal cancer patients into control group(50 cases,non-metastasized to liver)and observation group(30 cases,metastasized to liver),and enrolled into this study:to sample their elbow venous blood 10 mL,and get serum through centrifugation,then,by using fully automatic electrochemiluminescense device and enzyme assay apparatus to detect the levels of serum CA50,AFP,CEA,andβ2-MG;then,compared both groups’ levels of CA50,AFP,CEA,andβ2-MG,and those indexes’ positivity status,and evaluated diagnostic value of single index and their combination,analysed their relevant specificity,sensitivity,accuracy,and Uden coefficient.As results,in the levels of CA50,AFP,CEA,andβ2-MG observation group was all significantly higher than control group(P <0.05),and the positivity ratio of CA50,AFP,CEA and combination detection of above four indexes in observation group were all higher than that in control group(P <0.05),except forβ2-MG’s ratio of positivity did not having statistical difference between the two groups(P >0.05),though observation groups’ ratio being higher than control groups’ one;more than,the accuracy and Uden coefficients was most high.Results show that CEA,β2-MG,CA50 and AFP could be serviced as diagnostie indexes for colorectal cancer and its liver metastasis,and combination detection is beneficial to uplift this diagnotic capacity.
