血清肿瘤标志物联合检验诊断结直肠癌的价值

The Value of Serum Tumor Marker Combined Laboratory Exam.in the Diagnosing Colorectal Cancer

为探究血清肿瘤标志物联合检验诊断结直肠癌的价值,选取2017-2018年在我院治疗的67例结直肠癌患者为观察组,并以同期健康体检者67名为对照组,观察2组癌胚抗原(CEA)、糖类抗原(CA19-9、CA72-4)、细胞角蛋白19的可溶性片段(CYFRA211)的表达水平、不同分期血清肿瘤标志物水平以及检测灵敏度与特异性。结果显示,经检测后,观察组患者的CEA、CA19-9、CA72-4、CYFRA211水平显著高于对照组(P <0.05)。晚期结直肠癌患者CEA、CA19-9、CA72-4以及CYFRA211检测水平均高于早期,比较差异有统计学意义(P <0.05)。观察组患者CEA、CA19-9、CA72-4以及CYFRA211单一敏感性分别为66.98%、68.54%、66.21%和68.13%,联合检测敏感性为86.98%,单一检测特异性分别为86.98%、83.56%、79.98%和84.56%,联合检测特异性为89.56%,比较差异有统计学意义(P <0.05)。结果表明,血清肿瘤标志物联合检测可提高结直肠癌的诊断率,有助于早期与晚期的鉴别,为临床提供重要的治疗依据。

This study was to investigate the value of serum tumor marker combined laboratory exam.in the diagnosis of colorectal cancer,enrolled 67 patietns with colorectal cancer treated in author’s hospital(2017-2018 yr.)as observation group,at the same time,67 physical examinees as control group;then,observed and compared both groups’ levels of CEA,CA19-9,CA72-4,and CYFRA 211;and the levels of serum tumor markers at different stages,as well as the sensitivity and specificity of detection.As results,the levels of CEA,CA19-9,CA72-4,and CYFRA 211 in observation group were significantly higher than that in control group(P <0.05);in late patients above levels were higher than in early patients(P <0.05);in observation groups’ patients the sensitivity of single CEA,CA19-9,CA72-4,and CYFRA 211 detection was respectively 66.98%,68.54%,66.21% and 68.13%;while the sensitivity of their combination detection was86.98%,the difference was significant(P <0.05);the specificity of single one’s detection was respectively86.98%,83.56%,79.89%and 84.56%,while combination detections’ specificity was 89.56%,the difference was significant(P <0.05).Results show that serum tumor marker combined laboratory exam.can enhance diagnostic rate of colorectal cancer,and is helpful to differentiate early tumor and late one,thus provide important treatment basis clinically.
