个案管理模式在高血压脑出血患者术后护理中的应用

Application of Case Management Mode in Postoperative Nursing of Patients with Hypertensive Cerebral Hemorrhage

目的探讨个案管理模式在高血压脑出血患者术后护理中的应用效果。方法回顾性分析2018年2月—2019年5月于该院接受手术治疗的84例高血压脑出血患者的临床资料,依据干预模式的不同分为两组,对照组(常规护理,42例)和观察组(个案管理模式护理,42例),观察分析两组的生活能力及并发症发生情况。结果观察组的自理能力、括约肌控制、转移、行走、交流、社会认知、总分分别为(32.51±2.47)分、(8.64±1.19)分、(13.89±1.74)分、(11.92±1.36)分、(11.38±1.06)分、(17.18±1.57)分、(95.52±1.35)分,均较对照组的(23.74±2.05)分、(5.40±1.18)分、(10.24±1.06)分、(5.87±1.15)分、(9.28±1.01)分、(13.89±1.62)分、(68.42±1.96)分高,差异有统计学意义(t=17.707、12.530、11.610、22.014、9.295、9.451、73.795,P<0.05);观察组并发症总发生率(4.76%)较对照组(23.81%)低,差异有统计学意义(χ^(2)=6.222,P=0.013)。结论个案管理模式在高血压脑出血患者术后护理中的应用效果较好,可有效提高患者的生活自理能力,且并发症较少,安全性较高。

Objective To explore the application effect of case management model in postoperative nursing of patients with hypertensive cerebral hemorrhage.Methods A retrospective analysis of the clinical data of 84 patients with hypertensive intracerebral hemorrhage who underwent surgical treatment in the hospital from February 2018 to May 2019 was divided into 2 groups according to the different intervention modes,the control group(routine care,42 cases)and the observation group(case management mode nursing,42 cases),observe and analyze the ability of life and the occurrence of complications of the two groups.Results The self-care ability,sphincter control,transfer,walking,communication and social cognition of the observation group were(32.51±2.47)points,(8.64±1.19)points,(13.89±1.74)points,(11.92±1.36)points,(11.38±1.06)points,(17.18±1.57)points,(95.52±1.35)points,which were all compared with the control group’s(23.74±2.05)points,(5.40±1.18)points,(10.24±1.06)points,(5.87±1.15)points,(9.28±1.01)points,(13.89±1.62)points,(68.42±1.96)points were higher,the difference was statistically significant(t=17.707,12.530,11.610,22.014,9.295,9.451,73.795,P<0.05);the total incidence of complications in the observation group(4.76%)was lower than that in the control group(23.81%),and the difference was statistically significant(χ^(2)=6.222,P=0.013).Conclusion The case management model is effective in the postoperative nursing of patients with hypertensive intracerebral hemorrhage.It can effectively improve the patients'self-care ability,with fewer complications and higher safety.
