乳头状甲状腺微小癌和微小结节性甲状腺肿的CT鉴别诊断价值

Analysis of CT Differential Diagnosis Value between Papillary Thyroid Microcarcinoma and Micronodular Goiter

目的分析乳头状甲状腺微小癌与微小结节性甲状腺肿的CT鉴别诊断价值。方法选择2019年1月至2020年4月在本院接受CT诊断的甲状腺微小结节患者60例作为研究主体,根据病理学检验结果进行分组,即乳头状甲状腺微小癌组(PTMC组)与微小结节性甲状腺肿组(MNG组),仔细记录两组患者的CT征象与结节、腺体体积,分析鉴别诊断价值。结果相比MNG组,PTMC组混合钙化占比更低,瘤体不规则、细颗粒状钙化、结节单发、增强后边界模糊、边缘中断征占比更高,差异均有统计学意义(P<0.05);MNG组右叶体积、左叶体积均较PTMC组大,结节体积与峡部体积均较PTMC组小,差异均有统计学意义(P<0.05)。结论CT诊断技术在鉴别乳头状甲状腺微小癌与微小结节性甲状腺肿疾病中具有较高的应用价值。

Objective To analyze the value of CT differential diagnosis between papillary thyroid microcarcinoma and micronodular goiter.Methods Sixty patients with thyroid micronodules diagnosed by CT in our hospital from January 2019 to April 2020 were selected as the research subjects,and divided into papillary thyroid microcarcinoma group(PTMC group)and micronodular goiter group(MNG group)according to the pathological examination results.The CT signs,nodule and gland volume of the two groups were carefully recorded,and the application value was analyzed.Results Compared with MNG group,mixed calcification in PTMC group was lower,irregular tumor body,fine granular calcification,single nodule,blurred boundary and edge interruption sign were higher,and the differences were statistically significant(P<0.05).The volume of right lobe and left lobe in MNG group was larger than that in PTMC group,and the nodule volume and isthmus volume were smaller than those in PTMC group,the differences were statistically significant(P<0.05).Conclusion CT diagnostic technique has high application value in differentiating papillary thyroid microcarcinoma from micronodular goiter.
