摘要
目的探讨胱抑素、25(OH)D、尿酸对2型糖尿病周围神经病变的预测价值。方法选取2018年9月—2019年12月期间于该院就诊的180例糖尿病周围神经病变患者及无病变的2型糖尿病患者作为研究对象,分为无神经病变症状组、有神经病变症状组及无神经病变组,各60例,进行胱抑素C、25(OH)D、尿酸的检测,对比3组患者各项指标的差异性,建立logistic回归模型,进行多因素分析,判断各项指标与2型糖尿病周围神经病变的相关性。结果有神经病变症状组患者的胱抑素C、尿酸检测结果显著高于其他两组患者,无神经病变症状组患者的胱抑素C、尿酸检测结果高于无神经病变组,差异有统计学意义(P<0.05),有神经病变症状组患者的25(OH)D检测结果显著低于其他两组患者,无神经病变症状组患者的25(OH)D检测结果低于无神经病变组,差异有统计学意义(P<0.05),说明胱抑素、25(OH)D、尿酸与2型糖尿病周围神经病变的发生具有相关性。经过多因素Logistic回归分析,胱抑素、25(OH)D、尿酸是影响2型糖尿病周围神经病变发生的独立危险因素。结论2型糖尿病周围神经病变的防治工作中,可以根据胱抑素、25(OH)D、尿酸的检测结果进行预测,评估疾病的发生风险,便于及时、有效的采取防治措施,提高疾病的防治效果。
Objective To investigate the predictive value of cystatin,25(OH)D and uric acid in type 2 diabetic peripheral neuropathy.Methods 180 cases of diabetic peripheral neuropathy patients and type 2 diabetes patients who were treated in the hospital from September 2018 to December 2019 were selected as the research objects,and they were divided into groups without neuropathy symptoms and groups with neuropathy symptoms 60 cases in each group and 60 cases in the non-neuropathy group were tested for Cystatin C,25(OH)D,and uric acid.The differences in the indicators of the three groups were compared,a logistic regression model was established,and multi-factor analysis was performed to determine the indicators correlation with peripheral neuropathy of type 2 diabetes.Results The cystatin C and uric acid test results of the patients with neuropathy symptoms were significantly higher than those of the other two groups.The cystatin C and uric acid test results of the patients with no neuropathy symptoms were higher than those of the non-neuropathy group,and the difference was statistically significant(P<0.05),the 25(OH)D test results of the patients with neuropathy symptoms were significantly lower than those of the other two groups,and the 25(OH)D test results of patients with no neuropathy symptoms were lower than those of the non-neuropathy group,and the difference was statistically significant(P<0.05),indicating that cystatin,25(OH)D,uric acid and type 2 diabetic peripheral neuropathy were correlated.After multivariate logistic regression analysis,cystatin,25(OH)D and uric acid were independent risk factors affecting the development of type 2 diabetic peripheral neuropathy.Conclusion In the prevention and treatment of type 2 diabetic peripheral neuropathy,prediction can be made based on the detection results of cystatin,25(OH)D,and uric acid,and the risk of disease can be evaluated,so that timely and effective prevention and treatment measures can be taken to improve the disease control effect.
