摘要 目的探讨食管癌微创手术治疗食管癌患者的临床效果。方法回顾性分析我院2015年1月至2019年12月收治的80例食管癌患者的病历资料,根据手术方法的差异分为传统组以及微创组,每组40例。传统组进行McKeown手术,微创组进行腹部小切口联合胸腔镜McKeown食管切除术。比较两组患者的应激反应指标、生存质量、治疗效果、围手术期相关指标[疼痛程度(NRS评分)、手术时间、术中出血量、住院时间、并发症发生率]。结果术前,两组CRP、IL-6、肾素、肾上腺素、去甲肾上腺素比较,差异均无统计学意义(P>0.05);术后,两组CRP、IL-6、肾素、肾上腺素、去甲肾上腺素水平均较术前改善,且微创组显著低于传统组,差异均有统计学意义(P<0.05)。术前,两组生存质量评分比较,差异无统计学意义(P>0.05);术后,两组生存质量评分均较术前升高,且微创组显著高于传统组,差异有统计学意义(P<0.05)。微创组术后NRS评分、并发症发生率均低于传统组,术中出血量少于传统组,手术时间、住院时间短于传统组,差异均有统计学意义(P<0.05)。两组患者的治疗总有效率均为100.00%,差异无统计学意义(P>0.05)。结论采用微创手术治疗食管癌患者可减轻患者的应激反应,改善围手术期相关指标,提高其生存质量。 Objective To explore the clinical effect of minimally invasive surgery for esophageal cancer in patients with esophageal cancer.Methods A retrospective analysis of the medical records of 80 patients with esophageal cancer admitted to our hospital from January 2015 to December 2019 was divided into traditional group and minimally invasive group,with 40 cases in each group based on differences in surgical methods.The traditional group underwent McKeown surgery,and the minimally invasive group underwent a small abdominal incision combined with thoracoscopic McKeown esophagectomy.The stress response indexes,quality of life,treatment effect,perioperative related indexes[pain degree(NRS score),operation time,intraoperative blood loss,hospital stay,and complication rate]were compared between the two groups of patients.Results Before operation,the two groups of CRP,IL-6,renin,epinephrine,and norepinephrine were compared,and the difference was not statistically significant(P>0.05);after surgery,the two groups of CRP,IL-6,renin,adrenal glands The levels of hormone and norepinephrine were improved compared with preoperatively,and the minimally invasive group was significantly lower than the traditional group,the difference was statistically significant(P<0.05).Before operation,the difference in quality of life scores between the two groups was not statistically significant(P>0.05);after surgery,the quality of life scores of the two groups were higher than those before the operation,and the minimally invasive group was significantly higher than the traditional group,the difference was statistically significant Significance(P<0.05).The postoperative NRS score and complication rate of the minimally invasive group were lower than those of the traditional group,the amount of intraoperative blood loss was less than that of the traditional group,and the operation time and hospital stay were shorter than those of the traditional group.The differences were statistically significant(P<0.05).The total effective rate of the two groups of patients was 100.00%,the difference was not statistically significant(P>0.05).Conclusion The use of minimally invasive surgery to treat patients with esophageal cancer can alleviate the patient's stress response,improve related indicators during the perioperative period,and improve their quality of life.
