黄芩汤加减结合美沙拉嗪对溃疡性结肠炎(活动期)肠黏膜组织炎症因子的影响

Effect of Huangqin Decoction(黄芩汤)Combined with Mesalazine on Inflammatory Factors of Intestinal Mucosa in Patients with Ulcerative Colitis(Active Stage)

目的观察黄芩汤加减结合美沙拉嗪对溃疡性结肠炎(活动期)患者肠黏膜组织炎症因子的影响。方法选择2017年6月—2019年7月医院收治的溃疡性结肠炎患者116例,依据随机对照法将患者分为研究组和对照组,每组58例。对照组患者口服美沙拉嗪颗粒治疗,4 g/d,分3~4次服用。研究组在对照组治疗的基础上,口服黄芩汤加减治疗。两组均治疗2个月。比较两组治疗前后中医症候积分和临床疗效,比较两组治疗前后改善Mayo评分和肠黏膜组织TNF-α、IL-5、IL-6和IL-10水平。结果研究组治疗总有效率为91.38%(53/58),对照组治疗总有效率为77.59%(45/58),研究组治疗总有效率高于对照组(P<0.05);研究组治疗后腹痛、腹泻、血便和里急后重等中医症候积分低于对照组(P<0.05);研究组治疗后Mayo评分低于对照组(P<0.05);研究组治疗后肠黏膜组织炎症因子TNF-α、IL-5、IL-6水平低于对照组,IL-10水平高于对照组(P<0.05)。结论黄芩汤加减可有效的改善溃疡性结肠炎患者的临床症状,降低活动期患者的肠黏膜组织炎症因子水平,提高治疗疗效,值得临床进一步研究探讨。

Objective To observe the effect of Huangqin Decoction(黄芩汤)combined with mesalazine on inflammatory factors of intestinal mucosa in patients with ulcerative colitis(active stage).Methods A total of 116 patients with ulcerative colitis admitted to our hospital from June 2017 to July 2019 were selected.According to the randomized control method,the patients were divided into study group and control group,58 cases in each group.The patients in the control group were treated with mesalazine granules,4 g/d,three to four times.On this basis,Huangqin Decoction was used in the study group.Both groups were treated for 2 months.Before and after treatment,the score of TCM symptoms and clinical effect were compared between the two groups,and the Mayo score and the levels of TNF-α,IL-5,IL-6 and IL-10 in intestinal mucosa were improved before and after treatment.Results The total effective rate of the study group was 91.38%(53/58),and that of the control group was 77.59%(45/58).The total effective rate of the study group was higher than that of the control group(P<0.05).The levels of TNF-α,IL-5 and IL-6 were lower than those of the control group,and the levels of IL-10 were higher than those of the control group(P<0.05).Conclusion Huangqin Decoction can effectively improve the clinical symptoms of patients with ulcerative colitis,reduce the levels of inflammatory factors in the intestinal mucosa of patients with active stage and improve the therapeutic effect,which is worthy of further clinical study.
