雷公藤及其提取物治疗糖尿病肾病的作用机制及研究进展

Mechanism and research progress of Tripterygium wilfordii and its extracts in the treatment of diabetic nephropathy.

糖尿病肾病(diabetic nephropathy,DN)是一种常见的糖尿病微血管并发症。雷公藤作为传统中草药对DN治疗有显著疗效。雷公藤及其提取物能有效减少尿蛋白、保护肾功能。其相关药理机制是通过调节p38 MAPK、NF-κB、TGF-β、Wnt/β-catenin、Akt和Notch1等细胞内信号通路,从而发挥抗炎、抗氧化、抗肾小球硬化和纤维化、调节巨噬细胞浸润等作用。但雷公藤及其提取物易导致肝功能损害、胃肠道反应、月经失调和生殖系统问题等不良反应。在控制好用药剂量及疗程的前提下,雷公藤及其提取物是治疗DN的良好替代药物。本文就雷公藤及其提取物对DN治疗的相关作用机制及研究进展做一综述。

Diabetic nephropathy(DN)is a common microvascular complication of diabetes.As a traditional Chinese herbal medicine,Tripterygium wilfordii has remarkable curative effect on DN treatment.Tripterygium wilfordii and its extracts can effectively reduce urinary protein and protect renal function.Its pharmacological mechanism is through the regulation of p38 MAPK,NF-κB,TGF-β,Wnt/β-catenin,Akt and Notch1 intracellular signaling pathway,so as to play anti-inflammatory,antioxidant,anti-glomerular sclerosis and fibrosis,regulating macrophage infiltration.However,Tripterygium wilfordii and its extracts can easily lead to liver damage,gastrointestinal reactions,menstrual disorders and reproductive system problems.Tripterygium wilfordii and its extracts are good substitutes for DN under the premise of controlling dosage and course of treatment.In this paper,the mechanism of action and research progress of Tripterygium wilfordii and its extracts on DN treatment were reviewed.
