自我效能模式下的集中群组保健对高龄初产妇剖宫产术后纯母乳喂养率的影响

The Effect of Centralized Group Health Care under Self-efficacy Model on the Rate of Exclusive Breastfeeding after Cesarean Section in Elderly Primipara

目的探究自我效能模式下的集中群组保健对高龄初产妇剖宫产术后纯母乳喂养率的影响。方法在2018年1月—2020年1月期间进行实验观察,方便选取观察对象100例,均为在该院接受剖宫产的高龄初产妇。将产妇分为观察组和对照组,观察组(n=50)实施自我效能模式下的集中群组保健措施,对照组(n=50)采取常规护理。观察对比两组产妇的自我效能、母乳喂养率、泌乳情况等。结果护理前,观察组的自我护理能力评分、防御能力评分(12.4±1.3)分、(13.6±0.8)分,与对照组的(12.1±1.4)分、(13.7±0.9)分比较,差异无统计学意义(t=0.993、0.525,P>0.05);护理后,观察组为(17.6±1.5)分、(17.9±1.4)分,明显高于对照组(14.8±1.6)分、(15.1±1.2)分,差异有统计学意义(t=9.074、9.604,P<0.05);观察组母乳喂养率88.0%高于对照组62.0%,泌乳充足率86.0%高于对照组60.0%,差异有统计学意义(χ^(2)=9.013、8.574,P<0.05)。结论对高龄初产妇剖宫产产妇采取自我效能模式下的集中群组保健,能够显著提升产妇的自我护理能力、防御能力等,不仅能够提升其母乳喂养率,还能帮助许多缺乏经验的产妇掌握母乳喂养的正确姿势、注意事项等,有利于拉近亲子关系,促进家庭和谐,对于新生儿的健康也有益处,值得推广。

Objective To explore the effect of intensive group health care under self-efficacy model on the rate of exclusive breastfeeding after cesarean section in elderly primiparas.Methods The experimental observations were carried out from January 2018 to January 2020,and 100 cases of observation subjects were included,all of whom were elderly primiparas who received cesarean section in the hospital.The parturients were divided into observation group and control group.The observation group(n=50)implemented centralized group health care measures under the self-efficacy model;the control group(n=50)took routine care.Observed and compared the self-efficacy,breastfeeding rate,and lactation status of the two groups of women.Results Before nursing,the observation group's self-care ability score,defense ability score(12.4±1.3)points,(13.6±0.8)points,compared with the control group(12.1±1.4)points,(13.7±0.9)points,there was no statistically significant difference(t=0.993,0.525,P>0.05).After nursing,the observation group scored(17.6±1.5)points and(17.9±1.4)points,which were significantly higher than the control group(14.8±1.6)points and(15.1±1.2)points.The difference were statistically significant(t=9.074,9.604,P<0.05).The breastfeeding rate of the observation group was 88.0% higher than that of the control group 62.0%,and the lactation adequacy rate of 86.0% was higher than that of the control group 60.0%,the difference was statistically significant(χ^(2)=9.013,8.574,P<0.05).Conclusion Adopting centralized group health care under self-efficacy mode for senile primiparous women with cesarean section can significantly improve the self-care and defense capabilities of the mothers,not only improve the breastfeeding rate,but also help many inexperienced mothers to master the correct posture and precautions for breastfeeding,which are conducive to closer parent-child relationship and family harmony.It is also beneficial to the health of newborns and is worth promoting.
