彩超在乳腺良恶性肿瘤鉴别诊断中的应用价值 被引量:1

摘要 目的临床分析多普勒彩超技术应用于乳腺肿瘤良恶性诊断鉴别中的效果。方法回顾性分析2018年1月—2020年9月在该院确诊为乳腺肿瘤性疾病的44例患者的影像学检查资料,入组患者均接受手术病理检查,并在术前先后接受X线钼靶和高频彩超检查,以病理检查结果为金标准,比较两种检查方式对于肿瘤良恶性诊断价值;此外,依据病理检查对良、恶性肿瘤的诊断结果将患者分为良性组(n=30)、恶性组(n=14),比较两组患者彩超检查结果中的血流分级结果,探讨良、恶性肿瘤患者的彩超表现特点。结果以病理结果为金标准,钼靶X线检查结果中,恶性肿瘤诊断准确率为28.6%,良性肿瘤诊断准确率为83.3%,总体诊断准确率为65.9%,彩超检查结果:恶性肿瘤诊断准确率为85.7%,良性肿瘤诊断准确率为93.3%,总体诊断准确率为90.9%,X线钼靶、彩超对于乳腺肿瘤恶性的诊断准确率和总体诊断准确率比较,差异有统计学意义(χ^(2)=7.146、6.712,P<0.05);恶性组患者CDFI(血流分级)II级、III级患者比例也显著高于良性组,差异有统计学意义(Z=-4.834,P<0.05)。结论彩超技术不仅能准确反映良、恶性乳腺肿瘤病灶的结构形态学特点,还能评估肿瘤血流状况,在乳腺肿瘤疾病的临床诊断和鉴别中具有应用价值。 Objective To clinically analyze the effect of Doppler color Doppler ultrasound in the diagnosis and differentiation of benign and malignant breast tumors.Methods A retrospective analysis of the imaging data of 44 patients diagnosed with breast neoplastic disease in the hospital from January 2018 to September 2020.All patients in the group underwent surgical pathological examination and received X-rays before surgery.Mammography and high-frequency color Doppler ultrasound examinations use pathological examination results as the gold standard to compare the diagnostic value of the two examination methods for benign and malignant tumors.In addition,patients were divided into benign groups based on the diagnosis results of benign and malignant tumors by pathological examinations(n=30)and malignant group(n=14),compare the blood flow classification results in the color Doppler ultrasound examination results of the two groups of patients,and explore the color Doppler ultrasound performance characteristics of patients with benign and malignant tumors.Results Taking pathological results as the gold standard,in mammography X-ray examination results,the diagnostic accuracy rate for malignant tumors was 28.6%,the diagnostic accuracy rate for benign tumors was 83.3%,and the overall diagnostic accuracy rate was 65.9%.Color Doppler ultrasound examination results:diagnostic accuracy for malignant tumors The diagnostic accuracy rate of benign tumors was 85.7%,the diagnostic accuracy rate of benign tumors was 93.3%,and the overall diagnostic accuracy rate was 90.9%.Compared with the diagnostic accuracy of mammography and color Doppler ultrasound for malignant breast tumors,the difference was statistically significant(χ^(2)=7.146,6.712,P<0.05);the proportion of CDFI(blood flow classification)II and III patients in the malignant group was also significantly higher than that in the benign group,the difference was statistically significant(Z=-4.834,P<0.05).Conclusion Color Doppler ultrasound technology can not only accurately reflect the structural and morphological characteristics of benign and malignant breast tumors,but also evaluate the blood flow of the tumor.It has application value in the clinical diagnosis and differentiation of breast tumors.
