后腹腔镜与开放性肾根治性切除术治疗T2期肾癌的疗效差异Meta分析

Meta-analysis of the efficacy of retroperitoneal laparoscopic radical nephrectomy and open radical nephrectomy for T2 renal cell carcinoma

目的探讨后腹腔镜肾根治性切除术(RPN)与开放性肾根治性切除术(ORN)在12期肾癌治疗中的疗效差异,评价RPN的临床应用价值。方法检索国内外数据库中有关RPN与ORN治疗T2期肾癌的文献。提取文献数据、交叉核对并分析。结果共纳入分析文献11篇.肾癌患者738例。结果显示:与ORN比较,RPN手术时间短,术中出血量少、肠道功能恢复时间短,术后并发症发生率低.术后住院时间短,差异均有统计学意义(P<0.05)。结论在T2期肾癌的治疗中,RPN具有出血量少、肠道功能恢复早、术后并发症少及术后住院时间短等优势,值得临床上推广。

Objective To explore the clinical difference between retroperitoneoscopie radical nephreetomy(RPN)and open radical nephrectomy(ORN)in the treatment of stage T2 renal cell carei-noma(RCC),and evaluate the linical value of RPN in the treatment of RCC.Methods Literature related to the treatment of T2 stage RCC by RPN and ORN in domestic and foreign databases was searched.Literature data were extracted,cross-checked and analyzed.Results RPN has a long oper-ation time,less intraoperative blood loss,shorter intestinal function recovery time,lower postoperative complications,and shorter postoperative hospital stay(P<0.05).The difference is statistically signif-icant.Conclusions In the treatment of T2 stage RCC,RPN has the advantages of less bleeding,ear-ly recovery of intestinal function,less postoperative complications,and shorter postoperative hospital stay.Even if the operation time is long,it is a reasonable and ffective surgical method.
