输尿管软镜钬激光碎石术治疗直径1〜2cm肾结石的临床效果与安全性研究

Clinical effect and safety of ureteroscopic holniium laser lithotripsy in the treatment of diameter 1-2 cm renal calculi

目的探讨输尿管软镜钬激光碎石术治疗肾结石(1 cem≤结石直径≤2 cm)的临床效果与安全性。方法回顾性分析2014年1月至2018年1月在本院收治的96例肾结石患者,分为观察组和对照组,各48例。观察组采取输尿管软镜钬激光碎石术(FURL)治疗,对照组采取经皮肾镜取石术(PCNL)治疗,对比两组临床治疗效果,包括术后结石清除率、手术时间.术中估计失血量.术后30d内并发症发生率.术后住院天数,总住院天数。结果两组手术时间比较,差异无统计学意义(P>0.05)。观察组在术后血红蛋白下降、术后住院天数、总住院天数方面明显高于对照组,差异有统计学意义(P<0.05)。两组的结石残留及并发症发生率比较,差异均无统计学意义(P>0.05)。结论FURL的术中出血量、术后住院天数及住院总天数明显缩短,且术后并发症发生率低,是治疗肾结石(1 cm≤结石直径≤2 cm)的可替代手术方法。

Objective To investigate the efect and perioperative period of ureteroscopie hol-mium laser lithotripsy(FURL)in the treatment of kidney stones(1 cm≤stone diameter≤2 cm).Methods A retrospective analysis of 96 patients with kidney stones treated in our hospital from Janu-ary 2014 to January 2018 was randomly divided into observation group(48 cases)and control group(48 cases).The observation group received FURL,The patients in the control group were treated with percutaneous nephrolithotomy(PCNL).The clinical outcomes of the lwo groups were compared,in-eluding postoperative stone clearance rale,operation time,estimated intraoperative blood loss,inci-dence of complications within 30 days after operation,hospitalizaion days,total hospitalization days.Results The stone clearance rate of FURL group was lower than that of PCNL group.The diference was not statistically significant(P>0.05),The operation time,intraoperative blood loss,postopera-tive hospital stay and hospitalization days were shorter than those in PCNL group.The difference was statistically significant(P<0.05).Compared with patients in PCNL group,the compliceation rate in.FURL group was lower,but the diference was not stistically significant(P>0.05).Conclusions Compared with the classic PCNL,the stone clearance rate of FURL is slightly lower,while the opera-tion time,intraoperative blood loss,postoperative hospital stay and lotal hospital stay are significantly shortened,and the incidence of postoperative complications is low,is an altermative surgical treatment of kidney stones(1 cm≤stone diameter≤2 cm).
