364例先天性胫骨假关节患儿照顾者家庭抗逆力现状及影响因素分析

Current Status of Family Resilience of Caregivers of Patients with Congenital Pseudarthrosis of the Tibia and Its Influence Factors:A 364-case Study

目的调查先天性胫骨假关节患儿照顾者家庭抗逆力的现状,分析其影响因素。方法采用便利抽样法,选取在湖南省某三级甲等儿童医院进行治疗的364例先天性胫骨假关节患儿主要照顾者作为研究对象。采用一般资料问卷、中文版家庭复原力评估量表、创伤后成长评定量表对其进行问卷调查,Pearson相关分析家庭抗逆力与创伤后成长的相关性,采用多元线性回归分析家庭抗逆力的影响因素。结果本组先天性胫骨假关节患儿照顾者家庭抗逆力得分为(130.35±12.53)分,多元线性回归分析结果显示:照顾者与患儿关系、照顾者年龄、创伤后成长水平是先天性胫骨假关节患儿家庭抗逆力水平的影响因素(P<0.05)。结论本组先天性胫骨假关节患儿照顾者家庭抗逆力处于偏低水平,照顾者为父亲、年龄更大、创伤后成长水平越好的照顾者家庭抗逆力水平可能更好。提示医务人员应重点关注先天性胫骨假关节患儿照顾者的心理健康水平以及情绪的变化,有针对性、分层次的进行健康宣教,从而改善家庭抗逆力水平,促进患儿康复。

Objective To survey the current status of family resilience of caregivers of children patients with congenital pseudarthrosis of the tibia(CPT)and its influence factors.Methods With convenience sampling method,the main caregivers of 364 children with CPT treated in a tertiary grade A children’s hospital in Hunan Province were surveyed by using general information questionnaire,Chinese Family Resilience Assessment Scale and Posttraumatic Growth Inventory.Pearson correlation was used to analyze the correlation between family resilience and posttraumatic growth.Multiple linear regression was used to analyze the influencing factors of family resilience.Results Family resilience score of caregivers of children with CPT was 130.35±12.53.Multiple linear regression analysis showed that the relationship between caregivers and children,the age of caregivers and the level of posttraumatic growth were the influencing factors of family resistance in caregivers of children with CPT(P<0.05).Conclusion The family resilience of caregivers of children with congenital pseudarthrosis of the tibia is at a relatively low level.Families with father as the caregiver,those with older children and those with better post-traumatic growth may have better family resilience.Importance attached to mental health and emotional changes of caregivers and targeted health education are conducive to enhancing family resilience and the rehabilitation of children.
