朱良春痛风汤联合非布司他治疗间歇期痛风的临床疗效观察

Clinical observation of Zhu Liangchun gout decoction combined with febuxostat in the treatment of intermittent gout

目的:探讨朱良春痛风汤联合非布司他治疗间歇期痛风的疗效。方法:选取2018年1月—2020年8月汕头大学医学院第一附属医院间歇期痛风患者40例。其中男性38例,女性2例,年龄为(42.1±16.5)岁。将40例患者按治疗方法随机分为西药组和中西药组,每组各20例。中西药组给予朱良春痛风汤联合非布司他口服治疗,西药组给予非布司他口服治疗,比较治疗前后的症状积分、临床疗效以及尿酸浓度的下降情况,同时评价治疗的安全性。结果:中西药组治疗后症状积分为(1.5±1.0)分,低于西药组[(2.4±0.6)分,(P<0.05)]。西药组和中西药组临床疗效等级分布差异有统计学意义(U=129.500,Z=-2.102,P=0.036)。治疗8周后,中西药组血尿酸降幅为(191.33±69.21)μmol/L,高于西药组[(188.80±137.85)μmol/L,(P<0.05)]。西药组和中西药组在治疗前后,肝、肾功能各指标差异均无统计学意义(P>0.05),均未出现肝、肾功能损害。结论:朱良春痛风汤联合非布司他治疗间歇期痛风可以改善患者临床症状,临床疗效和降尿酸水平效果优于单用非布司他。

Objective:To investigate the efficacy of Zhu Liangchun gout decoction combined with febuxostat in the treatment of intermittent gout.Methods:From January 2018 to August 2020,40 patients with intermittent gout in the First Affiliated Hospital of Shantou University Medical College were selected.Among them,38 were males and 2 were females,and their age was(42.1±16.5)years old.According to the treatment method,40 patients were randomly divided into a western medicine group and a traditional Chinese and western medicine group,with 20 cases in each group.The traditional Chinese and western medicine group was treated with Zhu Liangchun gout decoction combined with febuxostat oral treatment,and the western medicine group was treated with febuxostat oral treatment.The symptom scores,clinical efficacy,and the decrease of uric acid concentration before and after treatment were compared,and the safety of the treatment was also evaluated.Results:The symptom score after treatment in the Chinese and western medicine group was(1.5±1.0)points,which was lower than the western medicine group[(2.4±0.6)points,(P<0.05)].There was a statistically significant difference in the clinical efficacy grade distribution between the western medicine group and the Chinese and western medicine group(U=129.500,Z=-2.102,P=0.036).After 8 weeks of treatment,the blood uric acid of the Chinese and western medicine group decreased by(191.33±69.21)μmol/L,which was higher than that of the western medicine group[(188.80±137.85)μmol/L,(P<0.05)].There was no significant difference in liver and kidney function between the western medicine group and the traditional Chinese and western medicine group before and after treatment(P>0.05),and there was no liver or kidney damage.Conclusion:Zhu Liangchun gout decoction combined with febuxostat in the treatment of intermittent gout can improve the clinical symptoms of patients,and the clinical efficacy and the effect of lowering uric acid level are better than that of febuxostat alone.
