摘要
华法林是人工瓣膜置换术后抗凝的唯一口服用药,目前采用国际标准化比值指导治疗,因其安全用药范围小,产生不良后果严重,故精准个体化治疗亟待深入研究。迄今已有大量文献报道了相关基因多态性对华法林的稳定治疗剂量有重要影响,但大多缺乏剂量预测模型的建立和必要的验证过程,部分建模文献也仅选用1~2个基因,且预测百分比低,由于人类不同个体之间存在差异过多,为更好的控制药物剂量,达到治疗效果,收集了295名患者用药资料,并分析患者8个基因位点,通过统计学对个体差异进行分析,建立了预测准确率在68%的华法林稳定剂量预测模型,从而更好地实现统计学在医学用药方向的应用,达到更佳的临床治疗效果。
Warfarin is the only oral anticoagulant after prosthetic valve replacement.At present the international standardized ratio is used to guide the treatment.Because of its small range of safe medication and serious adverse consequences,the precise individualized treatment needs to be further studied.So far a large number of literatures have reported that the related gene polymorphisms have an important impact on the stable therapeutic dose of warfarin,but most of them lack the establishment of dose prediction models and the necessary verification process.Some modeling literatures only select 1-2 genes,and the prediction percentage is low.Because there are too many differences between different human individuals,in order to better control the drug dose and achieve the therapeutic effect,the drug use data of 295 patients were collected,and 8 gene loci of patients were analyzed.Through statistical analysis of individual differences,a stable dose prediction model of warfarin with prediction accuracy of 68%was established,so as to better realize the application of statistics in the direction of medical medication and achieve better clinical treatment effect.
