婴儿捂热综合征机械通气指证研究与应用

Research and application of infant muggy syndrome mechanical ventilation indications

目的:探讨婴儿捂热综合征的通气指证。方法:选择2014年—2016年冬季30例婴儿捂热综合征患儿(重度)作为对照组,2017年—2019年冬季30例婴儿捂热综合征患儿(重度)作为观察组。对照组监测到患儿氧分压(PaO_(2))和二氧化碳分压(PaCO_(2))超过Ⅱ型呼吸衰竭标准时进行有创机械通气,观察组则是早期给予机械通气。比较两组患儿机械通气前和机械通气后第2天的危重评分、心率改善情况、乳酸水平以及治愈率、病死率和后遗症发生率。结果:观察组患儿心率及血气分析指标明显优于对照组,痊愈率高于对照组,病死率及后遗症发生率低于对照组,差异均有统计学意义(P<0.05)。结论:早期机械通气能迅速纠正婴儿捂热综合征(重度)患儿的全身组织及重要脏器的严重缺氧状态,减轻症状,降低病死率和后遗症发生率。

Objective:To explore the indications of ventilation in infant muggy syndrome.Methods:All 30 babies with severe infant muggy syndrome in winter from 2014 to 2016,were selected as the control group,and conventional mechanical ventilation indications were used for mechanical ventilation,30 infants in the winter of from 2017 to 2019 muggy syndrome(severe)were selected as the treatment group and given early mechanical ventilation.The criticality scores,improvement of main symptoms,lactic acid,etc.were compared between the two groups before mechanical ventilation and on the second day after mechanical ventilation.Finally,the cure rate and mortality rate of the two groups were compared.The incidence of sequelae in the two groups was compared after 1 year.Results:The main symptoms of children in the treatment group were improved faster than the control group,and the main results of blood gas analysis were significantly better than the control group.The recovery rate of the treatment group was significantly higher than the control group.Conclusion:Early mechanical ventilation can quickly correct the severe hypoxia state of the whole body and important organs in infants with the syndrome,reduce the symptoms,reduce the mortality and the incidence of sequelae.
