磁共振多参数定量技术在肝癌结节诊断中的研究

Multi-parameter quantitative magnetic resonance technology in diagnosis of liver cancer nodules

目的:采用磁共振多种参数定量技术比较肝癌不同阶段的结节特征,提高肝癌的早期诊断准确性。方法:选择2019年2月—2020年2月入院诊断的肝癌结节患者124例(186个结节),其中再生性结节(RN)36个、低级别异型增生性结节(LG-DN)30个、高级别异型增生性结节(HG-DN)40个和肝癌结节80个,分别采用3.0T MRI扩散加权成像(DWI)检测表观扩散系数(ADC值)、变容积肝脏采集(LAVA-Flex)技术检测脂肪分数(FF)和R^(2)值、钆塞酸对比剂增强检测增强率(ER)。结果:ADC和ER在RN,LG-DN,HG-DN和肝癌中逐渐降低(P<0.05);与RN和LG-DN相比,FF在HG-DN和肝癌中明显升高(P<0.05);与RN相比,R2值在LG-DN,HG-DN和肝癌中明显升高,LG-DN中最高(P<0.05)。结论:采用磁共振多种参数定量技术检测肝癌结节ADC,FF,R2和ER值,能够反映不同阶段的结节特征,提高肝癌的早期诊断准确性。

Objective:To compare the nodular characteristics of different stages of liver cancer by using multiple magnetic resonance parameter quantitative techniques to improve early diagnosis of liver cancer.Methods:A total of 124 patients(186 nodules)who were admitted to our hospital for diagnosis of liver cancer nodules from February 2019 to February 2020 were selected,of which 36 were regenerative nodules(RN)and low-grade dysplastic nodules(LG-DN)30,high-grade dysplastic nodules(HG-DN)40 and liver cancer nodules,3.0T MRI diffusion weighted imaging(DWI)was used to detect apparent diffusion coefficient(ADC value),variable volume liver collection(LAVA-Flex)technology was used to detect fat fraction(FF)and R^(2) values,and the acetic acid contrast agent enhanced detection enhanced rate(ER).Results:ADC and ER values were gradually decreased in RN,LG-DN,HG-DN and liver cancer(P<0.05).Compared with RN and LG-DN,FF values were significantly increased in HG-DN and liver cancer(P<0.05).Compared with RN,the R2 value was significantly increased in LG-DN,HG-DN and liver cancer,and the highest in LG-DN(P<0.05).Conclusion:Using multiple magnetic resonance quantitative parameters to detect ADC value,FF,R2 value and ER of liver cancer nodules can reflect the characteristics of nodules at different stages and improve the accuracy of early diagnosis of liver cancer.
