某大型综合医院心房颤动住院费用分析及管控建议

Analysis and Management Suggestions on Hospitalization Expenses of Atrial Fibrillation in a Large General Hospital

目的:利用西南某大型综合医院2011年-2019年病案首页数据,分析心房颤动住院费用现状,为相关管理人员提出费用控制管理建议。方法:采用R、Office Excel进行数据整理与分析。基于fisher检验、Student’s T检验和卡方检验等方法对房颤的基本费用情况进行分析,采用多元线性逐步回归模型对影响费用的相关因素进行分析。结果:患者自费费用呈现增长趋势,2011年至2019年期间增长了25995元;医保次均费用为先增高后下降趋势,2011年至2019年医保次均费用降低了6411元;房颤费用的可控性影响因素主要为住院时长和治疗方式。结论:房颤治疗次均费用逐渐增高,患者疾病负担逐渐增高,可通过加强住院时长管理、规范治疗行为和加强治疗方式成本效益分析等方式控制费用上涨。

Objective:Based on the data of the home page of medical records of a large general hospital in Southwest China from 2011 to 2019,this paper analyzes the current situation of hospitalization expenses of atrial fi brillation,and puts forward some suggestions for cost control management of relevant managers.Methods:Data sorting and analysis were carried out by R and Offi ce Excel.Based on Fisher test,Student's T test and chi square test,the basic cost of AF was analyzed,and the related factors aff ecting the cost were analyzed by using the multiple linear stepwise regression model.Results:From 2011 to 2019,the out of pocket expenses of patients increased by RMB 25995.From 2011 to 2019,the average cost of medical insurance decreased by RMB 6411.The controllable infl uencing factors of the cost of atrial fi brillation were the length of hospital stay and treatment methods.Conclusion:The average cost of atrial fi brillation treatment increases gradually,and the disease burden of patients increases gradually.We can control the cost increase by strengthening the management of hospitalization time,standardizing the treatment behavior and strengthening the cost-benefi t analysis of treatment methods.
