经导管主动脉瓣置换术临床应用及管理探讨

Discussion on Clinical Application and Management of Transcatheter Aortic Valve Replacement

目的追踪经导管主动脉瓣置换术治疗主动脉瓣狭窄的临床应用情况,与外科主动脉瓣置换术进行对比,就提升经导管主动脉瓣置换术管理水平提出建议。方法制定纳入排除标准,系统检索电子病例系统中2016—2019年符合入组条件的病例,进行特征分析。结果 (1)两组在手术时间长度、术后并发症方面的差异具有统计学意义;(2)两组在总费用、药费、诊断费、医用材料费方面的差异具有统计学意义。结论经导管主动脉瓣置换术可以有效地、较安全地应用于主动脉瓣狭窄患者,但其总体费用偏高。

Objective To track the clinical application of transcatheter aortic valve replacement for the treatment of aortic valve stenosis,to compare it with surgical aortic valve replacement,and to propose suggestions for improving the management of transcatheter aortic valve replacement.Methods The cases in electronic medical record system from 2016 to 2019 were screened by certain inclusion and exclusion criteria and feature analysis was conducted.Results(1)There were statistical differences in the length of operation and postoperative complications between the two groups;(2)There were statistical differences in thetotal cost,medicine fee,diagnosis fee,medical consumable fee between the two groups.Conclusion Transcatheter aortic valve replacement is effective and safe for the patients with aortic valve stenosis,but the total cost is relatively high.
