心源性脑梗死患者出血转化发生的影响因素分析

摘要 目的分析心源性脑梗死患者出血转化发生的影响因素。方法回顾性分析2019年2月至2020年3月我院收治的200例心源性脑梗死患者的临床资料,根据是否发生出血转化分为观察组(57例,发生出血转化)和对照组(143例,未发生出血转化)。采用单因素和Logistic多因素分析心源性脑梗死患者出血转化发生的影响因素。结果两组的年龄、性别、尿蛋白情况比较,差异无统计学意义(P>0.05);观察组基础疾病、心房颤动、大面积脑梗死、抗凝治疗、溶栓治疗占比均高于对照组(P <0.05)。经Logistic多因素回归分析显示,基础疾病、心房颤动、大面积脑梗死、抗凝治疗、溶栓治疗是心源性脑梗死患者出血转化发生的危险因素(OR>1, P <0.05)。结论基础疾病、心房颤动、大面积脑梗死、抗凝治疗、溶栓治疗是心源性脑梗死患者出血转化发生的危险因素,需及早采取针对性措施避免出血转化的发生。 Objective To analyze the influencing factors of hemorrhagic transformation in patients with cardiogenic cerebral infarction.Methods The clinical data of 200 patients with cardiogenic cerebral infarction admitted to our hospital from February 2019 to March 2020 were retrospectively analyzed,and divided into observation group(57 cases,hemorrhagic transformation)and control group(143 cases,without hemorrhagic transformation)according to the occurrence of hemorrhagic transformation.The influencing factors of hemorrhagic transformation in patients with cardiogenic cerebral infarction were analyzed by univariate and Logistic multivariate analysis.Results No statistical difference was found in the age,gender and urine protein between the two groups(P>0.05).The proportion of basic diseases,atrial fibrillation,large-area cerebral infarction,anticoagulant therapy,and thrombolytic therapy in the observation group were higher than those in the control group(P<0.05).Logistic multivariate regression analysis showed that the basic diseases,atrial fibrillation,large-area cerebral infarction,anticoagulation therapy,and thrombolytic therapy were the risk factors for hemorrhagic transformation in patients with cardiogenic cerebral infarction(OR>1,P<0.05).Conclusions Basic diseases,atrial fibrillation,large-area cerebral infarction,anticoagulation therapy,and thrombolytic therapy are the risk factors for hemorrhagic transformation in patients with cardiogenic cerebral infarction.It is necessary to take targeted measures as soon as possible to avoid hemorrhagic transformation.
