不同剂量利培酮治疗首发精神分裂症的临床疗效与安全性

Clinical efficacy and safety of different doses of risperidone in the treatment of first-episode schizophrenia

目的观察不同剂量利培酮治疗首发精神分裂症的临床疗效与安全性。方法选取2019年1月-2020年1月双峰县精神病医院收治的首发精神分裂症[阴性和阳性症状量表(PANSS)评分>60分]患者80例,采用随机抽签法分为观察组和对照组,各40例。2组均给予利培酮分散片治疗,观察组剂量为4 mg/d,对照组剂量为6 mg/d。比较2组患者治疗效果、治疗前后阴性及阳性症状评分变化及药物不良反应发生情况。结果治疗3个月后,观察组治疗总显效率为100.00%,高于对照组的87.50%(χ^(2)=5.333,P=0.021);2组PANSS评分均较本组治疗前明显降低(P<0.01),且观察组相较对照组降低幅度更大(P<0.01);治疗期间,观察组用药不良反应总发生率为10.00%,低于对照组的30.00%(χ^(2)=5.000,P=0.025)。结论首发精神分裂症患者采用不同剂量利培酮治疗的效果不同,且服药剂量与不良反应发生情况密切相关,小剂量利培酮用药疗效与安全性更高,值得进一步推广应用。

Objective To observe the clinical efficacy and safety of different doses of risperidone in the treatment of first-episode schizophrenia.Methods Eighty patients with first-episode schizophrenia(PANSS score>60)admitted to Shuangfeng County psychiatric hospital from January 2019 to January 2020 were selected.They were randomly divided into observation group and control group,40 cases in each group.Both groups were treated with risperidone dispersible tablets,the dose of the observation group was 4 mg/d,and the dose of the control group was 6 mg/d.The treatment effect,the changes of negative and positive symptom scores before and after treatment and the occurrence of adverse drug reactions were compared between the two groups.Results After 3 months of treatment,the total effective rate of the observation group was 100.00%,which was higher than 87.50%of the control group(χ^(2)=5.333,P=0.021);The PANSS scores of the two groups were significantly lower than those before treatment(P<0.01),and the decrease of the observation group was greater than that of the control group(P<0.01);During the treatment,the total incidence of adverse drug reactions in the observation group was 10.00%,which was lower than 30.00%in the control group(χ^(2)=5.000,P=0.025).Conclusion The effect of different doses of risperidone in the treatment of first-episode schizophrenia is different,and the dosage is closely related to the occurrence of adverse reactions.The efficacy and safety of low-dose risperidone is higher,which is worthy of further promotion and application.
