消痛方治疗原发性痛经的临床效果观察

Observation on the clinical effect of Xiaotong prescription in treating primary dysmenorrhea

目的观察消痛方治疗原发性痛经的临床疗效及对肿瘤坏死因子-α(TNF-α)、前列腺素F2α(PGF2α)的影响。方法选择2012年1月-2013年6月上海中医药大学附属岳阳中西医结合医院妇科门诊收治的原发性痛经患者60例为研究对象,采用随机数字表法分为中药组和西药组各30例。中药组服用消痛方,西药组服用对乙酰氨基酚片。比较2组临床疗效、治疗前后视觉模拟评分法(VAS)疼痛评分、血清TNF-α、PGF2α水平。结果中药组总有效率为90.00%,高于西药组的66.67%(χ^(2)=4.812,P=0.028)。治疗后,2组VAS疼痛评分均低于治疗前,且中药组低于西药组(P <0.01)。治疗后,2组血清TNF-α水平均低于治疗前,且中药组低于西药组(P <0.01)。治疗后,2组血清PGF2α水平均低于治疗前(P <0.05),但2组间比较差异无统计学意义(P> 0.05)。结论消痛方治疗原发性痛经效果较好,可通过抑制血清TNF-α、PGF2α的表达控制炎性反应。

Objective To observe the clinical efficacy of Xiaotong prescription in treating primary dysmenorrhea and its effect on tumor necrosis factor-α( TNF-α) and prostaglandin F2α( PGF2α) . Methods 60 patients with primary dysmenorrhea admitted to the gynecological clinic of Yueyang Hospital of Integrated Traditional Chinese and Western Medicine,Shanghai University of Traditional Chinese Medicine from January 2012 to June 2013 were selected as the research objects,and they were divided into Chinese medicine group and western medicine group by random number table method,30 cases in each group. The Chinese medicine group took Xiaotong prescription,and the western medicine group took acetaminophen tablets.The clinical efficacy,visual analogue scale( VAS) score of pain before and after treatment,the levels of serum TNF-α,PGF2αwere compared between the two groups. Results The total effective rate of the Chinese medicine group( 90. 00%) was higher than that of the western medicine group( 66. 67%)( χ^(2)= 4. 812,P = 0. 028). After treatment,the VAS pain scores in the two groups were lower than before treatment,and the Chinese medicine group was lower than the western medicine group( P <0. 01). After treatment,the levels of serum TNF-α in the two groups were lower than before treatment,and the Chinese medicine group was lower than the western medicine group( P < 0. 01). After treatment,the levels of serum PGF2αin the two groups were lower than before treatment( P < 0. 05),but there was no significant difference between the two groups( P >0. 05). Conclusion Xiaotong prescription is effective in treating primary dysmenorrhea,it can control the inflammatory response by inhibiting the expression of serum TNF-α and PGF2α.
