麻古相关性心肌病1例

Cardiomyopathy induced by Magu:a case report

1病例资料患者,男,33岁。因呼吸困难喘息1周入院。既往史:入院时患者否认吸毒史,追溯患者既往住院资料发现患者有吸服麻古史8年,2017年前因吸服麻古过量,引起急性心力衰竭(心衰)、肾功能不全。否认冠心病、高血压、糖尿病等慢性病史,否认扩张型心肌病家族史,否认外伤手术史,否认吸烟饮酒史,否认食物药物过敏史。

A 33-year-old man presented in our hospital with 7 days of breathlessness and wheezing.In 2017,he had acute heart failure and renal insufficiency after an overdose of Magu.During these years,he has taken drugs off and on.Echocardiography revealed enlargement of heart,left ventricular systolic dysfunction,right ventricular failure and EF only 13%.The man was diagnosed with exacerbations of chronic left ventricular dysfunction,dilated cardiomyopathy(DCM).After given a series of effective treatments,such as diuretic,antihypertensive and improving circulation,the male patient was markedly improved and then discharged.Drug history should be paid attention in younger DCM patients.We must be vigilant to chronic myocardial damage posed by drug abuse.
