全髋关节置换术治疗髋臼骨折后继发创伤性髋关节炎或股骨头坏死的中远期疗效分析

Analysis of medium-long-term effect of total hip arthroplasty in the treatment of traumatic arthritis of the hip or necrosis of the femoral head after acetabular fracture

目的:探讨全髋关节置换术治疗髋臼骨折后继发创伤性髋关节炎或股骨头坏死的中远期疗效。方法:2019年3月-2020年6月收治髋臼骨折后继发创伤性髋关节炎或股骨头坏死患者200例,按照不同治疗方式分为两组,各100例。对照组采用保守治疗;研究组采用全髋关节置换术治疗。比较两组中远期疗效。结果:研究组治疗3个月及1年后髋关节功能评分均高于对照组,差异有统计学意义(P<0.05);研究组治疗时间、术中出血量、术后引流量及住院时间均低于对照组,差异有统计学意义(P<0.05);研究组恢复优良率高于对照组,且并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:全髋关节置换术治疗髋臼骨折后继发创伤性髋关节炎或股骨头坏死,可显著提高中远期疗效,不仅有利于髋关节功能的恢复,也能降低并发症发生率。

Objective:To explore the medium-long-term effect of total hip arthroplasty in the treatment of traumatic arthritis of the hip or necrosis of the femoral head after acetabular fracture.Methods:From March 2019 to June 2020,200 patients with secondary traumatic arthritis of the hip or necrosis of the femoral head after acetabular fracture were selected and treated.They were divided into two groups according to different treatment methods,with 100 cases in each group.The control group received conservative treatment.The study group underwent total hip arthroplasty.The medium-long-term efficacy between the two groups was compared.Results:The hip function score of the study group was higher than the control group at 3 months and 1 year after treatment,with the differences were statistically significant(P<0.05).The treatment time,intraoperative blood loss volume,postoperative drainage volume and length of hospital stay in the study group were all lower than the control group,with the differences were statistically significant(P<0.05).The recovery good rate of the study group was higher than the control group,and the incidence of complications was lower than the control group,with the differences were statistically significant(P<0.05).Conclusion:Total hip arthroplasty for the treatment of traumatic hip arthritis or necrosis of femoral head after acetabular fracture can significantly improve the medium-long-term efficacy,it is not only beneficial to the recovery of hip function,but also can reduce the incidence of complications.
