医院-社区-家庭一体化服务模式在慢病家庭管理中的应用

Application of hospital-community-family integrated service model in family management of chronic diseases

目的:探讨在慢病管理中,医院-社区-家庭一体化服务模式的重要价值。方法:选取2020年5月-2020年6月社区慢病患者86例,随机分为两组,各43例。对照组采用常规管理;观察组行医院-社区-家庭一体化服务模式。比较两组干预效果。结果:观察组用药依从性评分增高情况优于对照组,观察组健康知识知晓率、生活质量评分及自我管理行为评分均明显高于对照组,观察组血压水平降低幅度高于对照组,差异有统计学意义(P<0.05)。结论:在慢病管理中,常规家庭式管理模式效果明显,医院-社区-家庭一体化服务模式效果更佳。

Objective:To explore the important value of hospital-community-family integrated service model in chronic disease management.Methods:From May 2020 to June 2020,86 community patients with chronic diseases were selected,they were randomly divided into the two groups with 43 cases in each group.The control group used routine management.The observation group used hospital-community-family integrated service model.The intervention effects of the two groups were compared.Results:The score of medication compliance in the observation group was better than that in the control group,the awareness rate of health knowledge,quality of life score and self-management behavior score in the observation group were significantly higher than those in the control group,and the decrease of blood pressure in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:In the management of chronic disease,the effect of conventional family management mode is obvious,and the effect of hospital-community-family integrated service model is better.
