血小板参数随增龄变化模式

Change Patterns of Platelet-Related Parameters with Increasing Age

目的建立实验室血小板相关参数的参考值,探讨血小板参数的影响因素。方法通过多阶段分层抽样的方法抽取298例样本,其中男142例,女156例;年龄10~90岁。Sysmex-XS 800i全自动血液分析仪测定血常规,并应用严格的质控措施保障数据准确。结果血小板相关参数间具有较强的相关性,其中血小板计数(platelet count,PLT)和血小板压积(plateletcrit,PCT)高度相关(r=0.901,P<0.001),血小板分布宽度(platelet distribution width,PDW)、平均血小板体积(mean platelet volume,MPV)和大血小板比例(large cell ratio,LCR)间存在强相关性(r>0.95,P<0.001)。血小板参数与性别无关(P>0.05),但PLT及PCT与年龄呈线性负相关(PLT&年龄,r=-0.195,P=0.001;PCT&年龄,r=-0.174,P=0.003),随着年龄增长,PLT和PCT逐渐降低。PDW、MPV及LCR与年龄呈非线性关系,呈单峰分布,随年龄增长逐渐增加,在50~60岁达到峰值,随后下降。结论血小板及其相关参数随增龄有不同的变化模式,有必要建立不同年龄段的血小板相关参数的参考范围。

Objective To establish platelet-related parameters’reference range in laboratory and explore the influence factors of platelet parameters.Methods A total of 298 samples were selected by multi-stage stratified sampling method,including 142 males and 156 females,aged from 10 to 90 years old.Blood routine was determined by Sysmex XS 800i automatic hematology analyzer,and strict quality control measures were applied to ensure the accuracy of data.Results There was a strong correlation among platelet parameters.Platelet count(PLT)was highly correlated with plateletcrit(PCT)(r=0.901,P<0.001),and there was a strong correlation among platelet distribution width(PDW),mean platelet volume(MPV)and large cell ratio(LCR)(r>0.95,P<0.001).Platelet parameters were not related to gender(P>0.05).Plt and pct were negatively correlated with age(PLT&age,r=-0.195,P=0.001;PCT&age,r=-0.174,P=0.003).PLT and PCT decreased with age.PDW,MPV and LCR had a non-linear relationship with age,showing a unimodal distribution,gradually increasing with age,reaching the peak at 50-60 years old,and then decreasing.Conclusion As the age increases,platelet-related parameters have different changing patterns.It is necessary to establish platelet-related parameters’reference ranges for different age groups.
