经皮椎体后凸成形术治疗胸腰椎骨质疏松性骨折的临床价值分析

Clinical Value of Percutaneous Kyphoplasty in the Treatment of Thoracolumbar Osteoporotic Fractures

目的:分析经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗胸腰椎骨质疏松性骨折患者的临床疗效,并研究其对患者疾病预后的影响。方法:选取78例在本院2018年1月-2020年1月就诊的胸腰椎骨质疏松性骨折患者,依据随机数字表法分为两组。对照组39例给予经皮椎体成形术治疗,观察组39例给予PKP治疗。观察两组手术前后椎体结构和功能及术后6个月并发症发生情况、疼痛和生活质量。结果:观察组术后1周Cobb角、伤椎前缘高度、ODI均优于对照组,差异有统计学意义(P<0.05)。观察组术后6个月VAS评分和并发症发生率明显低于对照组(P<0.05);Barthel指数评分明显高于对照组(P<0.05)。结论:采用PKP方案对胸腰椎骨质疏松性骨折患者进行治疗,能够促进患者损伤脊柱高度恢复,同时显著改善其生活质量,促进患者预后。

Objective:To analyze the clinical efficacy of percutaneous kyphoplasty(PKP)in the treatment of thoracolumbar osteoporotic fractures,and to study the effect on the prognosis of patients with disease.Method:Seventy-eight patients with thoracolumbar osteoporotic fractures who treated in our hospital from January 2018 to January 2020 were selected and randomly divided into two group according to different treatment methods.39 cases in the control group were treated with percutaneous vertebroplasty,and 39 cases in the observation group were treated with PKP.The vertebral structure and function,complications,pain and quality of life were observed before and after operation.Result:Cobb angle,anterior height of injured vertebrae and ODI of observation group were better than those of control group one week after operation,and the differences were statistically significant(P<0.05).The VAS score after 6 months of surgery and the incidence of complications in the observation group were significantly lower than those in the control group,and the Barthel index score was significantly higher than that in the control group(P<0.05).Conclusion:The application of PKP for the treatment of thoracolumbar osteoporotic fractures can promote the recovery of the injured spine height,at the same time,it can significantly improve the quality of life,and promote the prognosis of the patients.
