不典型主动脉夹层患者多层螺旋CT影像特征分析

Analysis of Multi-slice Spiral CT Imaging Features in Patients with Atypical Aortic Dissection

目的:探讨不典型主动脉夹层(AAD)患者多层螺旋CT(MSCT)影像特征。方法:回顾性收集本院2017年1月-2019年12月收治的AAD患者18例,患者均行MSCT平扫、增强扫描,获得平扫征象。结果:Stanford分型A型5例,B型13例,患者主动脉壁均呈环形或新月形增厚,平扫呈高密度,无增强现象。4例内膜渗漏,3例穿透性溃疡,4例病变区域主动脉增宽,2例管腔内径变窄,4例管腔内径增宽。结论:经MSCT平扫发现,AAD患者的影像学特征较为显著,主要是主动脉壁呈环形或新月形增厚,还可伴有内膜钙化内移、穿透性溃疡、内膜渗漏等特征,具有较高临床诊断价值。

Objective:To investigate the imaging features of multi-slice spiral CT(MSCT)in patients with atypical aortic dissection(AAD).Method:A total of 18 AAD patients treated in our hospital from January 2017 to December 2019 were retrospectively collected.All patients underwent MSCT plain scan and enhanced scan to obtain plain scan signs.Result:According to Stanford classification,there were 5 cases of type A and 13 cases of type B.The aortic wall of the patients was thickened in a ring or crescent shape,and the plain scan showed high density without enhancement.4 cases of intimal leakage,3 cases of penetrating ulcers,4 cases of lesion area aorta widening,2 cases of lumen inner diameter narrowing,4 cases of lumen inner diameter widening.Conclusion:After MSCT plain scan finding that the imaging characteristics of AAD patients are more significant,mainly because the wall of the aorta is thickened in a ring or crescent shape,it can also be accompanied by intimal calcification,penetrating ulcers,intimal leakage and other characteristics,which has high clinical diagnostic value.
