个案管理对肇庆市社区高风险严重精神障碍患者疾病控制及社会功能的效果研究

The Influence of Case Management for Disease Control and Social Function of High-risk Patients with Severe Mental Disorders in Community of Zhaoqing City

目的:探究个案管理对肇庆市社区高风险严重精神障碍患者疾病控制及社会功能的效果。方法:选取2018年1月-2019年6月肇庆市社区的60例高风险严重精神障碍患者,随机分为对照组(社区管理组)和观察组(个案管理组),每组30例。比较两组干预前后用药态度量表(DAI)、简明精神量表(BPRS)、社会功能缺陷筛选量表(SDSS)评分及干预效果。结果:干预前两组DAI评分及服药态度较好率比较差异无统计学意义(P>0.05),干预后观察组DAI评分及服药态度良好率均优于对照组,差异均有统计学意义(P<0.05);干预前两组BPRS评分、SDSS评分比较差异无统计学意义(P>0.05),干预后观察组BPRS评分、SDSS评分均优于对照组,差异均有统计学意义(P<0.05);干预后观察组病情好转率高于对照组,复发住院率低于对照组,差异均有统计学意义(P<0.05)。结论:个案管理有助于高风险严重精神障碍患者疾病控制及社会功能的改善,在社区高风险严重精神障碍患者中的应用价值较高。

Objective:To investigate the influence of case management for disease control and social function of high-risk patients with severe mental disorders in community of Zhaoqing City.Method:A total of 60 high-risk patients with severe mental disorders in community of Zhaoqing City from January 2018 to June 2019 were selected,and were randomly divided into the control group(community management group)and the observation group(case management group),30 cases in each group.The scores of medication attitude scale(DAI),concise mental scale(BPRS)and social disability screening scale(SDSS)before and after intervention were compared between the two groups.Result:Before intervention,there were no significant differences in DAI score and rate of good attitude towards medication between the two groups(P>0.05),after intervention,the DAI score and rate of good attitude towards medication in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).Before intervention,there were no significant differences in BPRS score and SDSS score between the two groups(P>0.05),after intervention,BPRS score and SDSS score in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).After intervention,the improvement rate in the observation group was higher than that in the control group,and the rate of relapse and hospitalization was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:The case management is helpful for the disease control and improvement of social function of high-risk patients with severe mental disorders,so its application value in the high risk patients with severe mental disorders in community is higher.
