超声追踪技术联合超声心动图在妊娠期高血压孕妇心功能中的应用价值 被引量:4

摘要 目的研究超声追踪技术联合超声心动图在妊娠期高血压(HIP)孕妇心功能中的应用价值。方法前瞻性选择2018年1~12月北京市平谷区医院妇产科接受治疗的70例HIP孕妇作为研究组,同期健康体检者60例作为对照组。采用超声心动图及超声追踪技术2种方法检查患者左心室功能,比较2组心脏形态指标、左心室收缩和舒张功能指标、左心室旋转指标、左心室运动情况。结果研究组左心室舒张末期内径(LVDd),左心室收缩末期内径(LAD)、左心室后壁舒张期厚度(LVPWTd)、室间隔舒张末期厚度(IVSTd)、左心室重量指数(LVMI)及相对室壁厚度(RWT)值均显著高于对照组,差异有统计学意义(P <0.05)。研究组舒张早期最大流速/舒张晚期最大速度比值(E/A)、舒张早期快速充盈分数(RFF)及心脏指数(CI)值显著小于对照组,左心室射血分数(LVEF)值显著高于对照组,差异有统计学意义(P <0.05);2组心输出量(CO)值比较差异无统计学意义(P> 0.05)。研究组心底水平旋转角度峰值(Prot-MV)、心底水平旋转角度达峰时间(Pti-MV)值显著低于对照组,心尖水平旋转角度峰值(Prot-AP)、心尖水平旋转角度达峰时间(Pti-AP)、心底水平旋转角度达峰时间百分比(Perc-Pti-MV)值显著高于对照组,差异有统计学意义(P<0.05);2组心尖水平旋转角度达峰时间百分比(Perc-Pti-AP)值比较差异无统计学意义(P> 0.05)。研究组舒张早期左心室侧壁速度(ELV)和位移(ELD)值显著小于对照组,舒张晚期左心室侧壁速度(ALV)和位移(ALD)值显著高于对照组,差异有统计学意义(P <0.05)。结论超声追踪技术联合超声心动图能准确评价HIP孕妇高血压孕妇心功能。 Objective To investigate value of tracking technique combined with echocardiography in cardiac function of hypertension in pregnancy(HIP).Methods From January to December 2018,70 cases of HIP(study group)and 60 cases of healthy women(control group)admitted to Beijing Pinggu Hospital were enrolled.The tracking technique and echocardiography were used to examine the left ventricular function of the patients,then indicators including cardiac morphological index,left ventricular systolic index,diastolic function index,left ventricular rotation index and left ventricular movement were compared.Results The values of left ventricular end diastolic diameter(LVDd),left ventricular end systolic diameter(LAD),left ventricle posterior wall of thickness diastolic(LVPWTd),interventricular septal end-diastolic thickness(IVSTd),left ventricular weight(LVMI)and relative wall thickness(RWT)in the study group were significantly higher than those in the control group(P<0.05).The E/A,fast filling fraction(RFF),and cardiac index(CI)values in the study group were significantly lower than those in the control group(P<0.05).The left ventricular ejection fraction(LVEF)value of the study group was significantly higher than that of the control group(P<0.05),with no difference in cardiac output(CO)values(P>0.05).The Prot-MV and Pti-MV values in the study group were significantly lower than those in the control group(P<0.05).The Prot-AP,Pti-AP and Perc-Pti-MV values in the study group were significantly higher than those in the control group(P<0.05),with no difference in Perc-Pti-AP values(P>0.05).The ELV and ELD values of the study group were significantly lower than those of the control group(P<0.05).The ALV and ALD values of the study group were significantly higher than those of the control group(P<0.05).Conclusion Ultrasound tracking technology combined with echocardiography can accurately evaluate the cardiac function of pregnant women with HIP in pregnant women.
