临床药师实施临床路径合理用药监管效果评价

Effect Evaluation of Clinical Pharmacists’Implementation of the Supervision of Rational Drug Use in the Clinical Pathway

目的评价临床药师实施临床路径合理用药监管的效果。方法选取四川省宜宾市第一人民医院2018年1月至2019年12月进入股骨粗隆间骨折临床路径并行内固定手术的患者200例,按是否进行合理用药监管分为对照组(2018年1月至12月)和干预组(2019年1月至12月),各100例。两组患者均给予统一的规范检查和治疗,临床药师全程参与干预组患者的临床路径合理用药监管,对路径用药进行技术干预和行政干预;对照组未进行任何干预。比较两组的不合理用药情况、平均住院时间、平均住院费用及平均药品费用。结果与对照组比较,干预组患者的平均住院时间更短,但差异无显著性(P>0.05);干预组患者的平均住院费用、平均药品费用、不合理用药发生率均明显降低(P<0.05)。结论临床药师实施临床路径合理用药监管,可降低医疗成本,提高用药合理率。

Objective To evaluate the effect of clinical pharmacists’implementation of the supervision of rational drug use in the clinical pathway.Methods From January 2018 to December 2019,200 patients with femoral intertrochanteric fracture underwent internal fixation in the clinical pathway in The First People’s Hospital of Yibin were selected and divided into the control group(January to December2018)and the intervention group(January to December 2019)according to whether implementation of the rational drug use or not,100 cases in each group.The patients in the two groups were given unified normative examination and treatment,clinical pharmacists participated in the whole process of clinical pathway and given supervision of rational drug use for patients in the intervention group,and carried out technical intervention and administrative intervention on medication in the clinical pathway.The patients in the control group were not given any intervention.The irrational drug use,average length of stay,average hospitalization cost and average drug cost were compared between the two groups.Results Compared with those in the control group,the average length of stay in the two groups was shorter,but the difference was not significant(P>0.05),the average hospitalization cost,average drug cost and the incidence of irrational drug use in the intervention group were significantly lower(P<0.05).Conclusion Clinical pharmacists can reduce the medical cost and improve rational drug use by implementing the supervision of rational drug use in the clinical pathway of patients with femoral intertrochanteric fracture.
