摘要
目的探讨高频超声联合血清细胞角蛋白19片段(CK19)、抗甲状腺球蛋白抗体(TGAb)对甲状腺乳头状癌淋巴结转移的预测价值。方法选取2017年1月至2020年1月重庆市东南医院收治的81例甲状腺乳头状癌患者,根据患者术后病理学检查是否出现淋巴结转移分为阳性组(35例)和阴性组(46例)。2组患者均接受高频超声和血清CK19、TGAb检测并进行比较,同时分析其检测价值。结果81例甲状腺乳头状癌患者,出现淋巴结转移35例,未出现淋巴结转移46例;2组患者在淋巴结数量、淋巴结最大径、淋巴结形态、淋巴结内部回声、淋巴结钙化情况这几方面比较,差异有统计学意义(P<0.05);阳性组血清CK19、TGAb水平高于阴性组(P<0.05);高频超声联合血清CK19、TGAb检测淋巴结转移的诊断效能更高。结论高频超声联合血清CK19、TGAb对甲状腺乳头状癌淋巴结转移的预测价值较高,有效提高了其检测灵敏度和特异度。
Objective To investigate the predictive value of high-frequency ultrasound combined with serum cytokeratin 19 fragment(CK19)and anti-thyroglobulin antibody(TGAb)in the prediction of lymph node metastasis in papillary thyroid carcinoma.Methods A total of 81 patients with papillary thyroid carcinoma admitted to the hospital from January 2017 to January 2020 were selected and divided into a positive group(35 cases)and a negative group(46 cases)according to whether the patients had lymph node metastasis in postoperative pathological examination.Both groups of patients received high-frequency ultrasound and serum CK19 and TGAb levels were tested and compared,and the detection value was analyzed at the same time.Results In 81 patients with papillary thyroid carcinoma,35 cases had lymph node metastasis and 46 cases did not have lymph node metastasis.The two groups of patients were compared in terms of the number of lymph nodes,the maximum diameter of the lymph node,the morphology of the lymph node,the internal echo of the lymph node,and the calcification of the lymph node,and the differences were statistically significant(P<0.05).The serum CK19 and TGAb levels of the positive group were higher than those of the negative group(P<0.05).High-frequency ultrasound combined with serum CK19,TGAb was more effective in the diagnosis of lymph node metastasis.Conclusion High-frequency ultrasound combined with serum CK19 and TGAb have a higher predictive value for lymph node metastasis in papillary thyroid carcinoma.
