摘要
目的分析MRI和CT动态增强扫描在原发性肝癌诊断中的应用价值,为临床选择合理的诊断方法提供参考。方法选取2016年11月~2019年11月我院收治的疑似为原发性肝癌的64例患者作为研究对象,均行MRI和CT动态增强扫描检查,比较两种方法诊断结果,MRI和CT表现及其在原发性肝癌诊断中的灵敏度、特异性、准确度、阳性预测值、阴性预测值。结果CT增强扫描表现:病灶强化密度下降;MRI增强扫描表现:病灶以类圆形为主,部分病灶为分叶状,病灶在T2WI上呈高信号,在T1WI上呈低信号,肿瘤边界较为清晰。MRI在原发性肝癌诊断中的灵敏度、特异性、准确度、阳性预测值、阴性预测值分别为92.45%、63.64%、90.63%、92.45%、63.63%,CT动态增强扫描在发性肝癌诊断中的灵敏度、特异性、准确度、阳性预测值、阴性预测值分别为86.79%、72.73%、84.38%、93.88%、53.33%,二者比较,差异无统计学意义(P>0.05)。结论MRI和CT动态增强扫描在原发性肝癌诊断中均具有较高的应用价值,临床可根据患者的病情及经济状况选择合理的诊断方法。
Objective To analyze the application value of MRI and CT dynamic-enhanced scanning in the diagnosis of primary liver cancer,and to provide references for clinical selection of reasonable diagnostic methods.Methods 64 patients with suspected primary liver cancer admitted to our hospital from November 2016 to November 2019 were selected as the research objects.All of them underwent MRI and CT dynamic enhancement scans.The diagnosis results of the two methods were compared.MRI and CT manifestations and their sensitivity,specificity,accuracy,positive predictive value,and negative predictive value in the diagnosis of primary liver cancer.Results CT enhanced scan showed that the density of the lesions decreased;enhanced MRI scans showed that the lesions were mainly round-shaped,and some of the lesions were lobulated.The lesions showed high signal on T2WI and low signal on T1WI,with clear tumor boundaries.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of MRI in the diagnosis of primary liver cancer are 92.45%,63.64%,90.63%,92.45%,and 63.63%,respectively.CT dynamic enhancement scans are used in the diagnosis of liver cancer.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of the two were 86.79%,72.73%,84.38%,93.88%,and 53.33%,respectively.There was no statistically significant difference between the two(P>0.05).Conclusion Both MRI and CT dynamic enhanced scanning have high application value in the diagnosis of primary liver cancer.Clinically,a reasonable diagnosis method can be selected according to the patient's condition and economic status.
