微钢板内固定术与克氏针穿髓内固定术治疗掌指骨开放性骨折的效果评价

Evaluation of the Effect of Microplate Internal Fixation and Kirschner Wire Intramedullary Fixationon Open Metacarpal and Phalangeal Fractures

目的比较微钢板内固定术与克氏针穿髓内固定术治疗掌指骨开放性骨折的临床效果。方法选取2017年6月~2019年8月因掌指骨开放性骨折于我院治疗的56例患者作为研究对象,根据治疗方案不同分为观察组(31例)与对照组(25例),对照组予克氏针穿髓内固定术治疗,观察组采用微钢板内固定术治疗。比较两组临床疗效、手术时间、术中出血量、骨折愈合时间、住院时间、术后生活质量及并发症发生情况。结果观察组总有效率高于对照组(93.55%vs 72.00%),差异有统计学意义(P<0.05);观察组手术时间、骨折愈合时间及住院时间均短于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05);观察组术后生活能力、自理能力、运动能力、疼痛与不适感、焦虑及抑郁感评分均高于对照组,差异有统计学意义(P<0.05)。结论微钢板内固定术治疗掌指骨开放性骨折的临床效果突出,手术时间短,术后恢复快,安全性良好,且可改善患者生活质量,值得临床应用。

Objective To compare the clinical effects of microplate internal fixation and Kirschner wire intramedullary fixation in the treatment of open metacarpal and phalangeal fractures.Methods 56 patients who were treated in our hospital for open metacarpal and phalangeal fractures from June 2017 to August 2019 were selected as the research objects.According to different treatment options,they were divided into observation group(31 cases)and control group(25 cases).The control group was treated with Kirschner wire intramedullary fixation,and the observation group was treated with microplate internal fixation.The clinical efficacy,operation time,intraoperative blood loss,fracture healing time,hospital stay,postoperative quality of life and complications were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(93.55%vs 72.00%),the difference was statistically significant(P<0.05);The operation time,fracture healing time and hospitalization time of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05);The incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05);The postoperative living ability,self-care ability,exercise ability,pain and discomfort,anxiety and depression scores of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05).Conclusion The clinical effect of microplate internal fixation in the treatment of open fractures of the metacarpal and phalanges is outstanding,the operation time is short,the postoperative recovery is fast,the safety is good,and the quality of life of patients can be improved,and it is worthy of clinical application.
