食管胃结合部腺癌的定义、分型、分期及外科治疗进展

Definition,Classification and Staging of Adenocarcinoma of Esophagogastric Junction and Progress of Surgical Treatment

食管胃结合部腺癌(AEG)的发病率在国内外均呈上升趋势。因其肿瘤位于食管胃交界处以及淋巴结转移既可向上又可向下等特殊性,国内外学者们对于该病的定义、分型、分期的认识均存在一定争议。基于对该肿瘤定义、分型、分期的认识,手术路径、手术切除范围、淋巴结清扫范围、消化道重建方式的选择也存在一定争议,这使得患者难以获得最佳的受益。目前,有关AEG的研究较多,外科治疗是其主要治疗方法。未来,应更新对AEG定义、分型、分期的认识,以进一步明确该肿瘤的外科治疗选择。

The incidence of adenocarcinoma of esophagogastric junction(AEG)is on the rise at home and abroad.Due to the specificity of the location at the esophagogastric junction and the lymph node metastasis possibility of both up and down,there are still controversies about the definition,classification and staging of the disease in the field at home and abroad.Based on the understanding of the definition,classification and staging of the tumor,there are also some controversies about the choice of surgical approach,surgical resection range,lymph node dissection range and digestive tract reconstruction mode,which makes it difficult for the patients to obtain the best benefit.At present,there are many researches on AEG,and surgical treatment is the main treatment.In the future,the understanding of AEG definition,classification and staging should be updated,so as to further clarify the of surgical treatment options.
