牙冠延长术后桩核修复在牙体缺损至龈下残根残冠患者中的应用观察

Observation on the application of post-core restoration with crown lengthening in patients withtooth defect to subgingival residual root and residual crown

目的观察牙冠延长术后桩核修复在牙体缺损至龈下残根残冠患者中的应用效果。方法选取我院2016年10月~2019年11月期间接收的牙体缺损至龈下残根残冠患者140例,根据随机数字表法分成研究组(70例)与对照组(70例),其中对照组接受直接桩核冠修复治疗,研究组接受牙冠延长术后桩核修复治疗。比较两组临床疗效、美观满意度、治疗前与治疗后龈沟液中炎性因子[一氧化氮(NO)、白细胞介素-1β(IL-1β)、前列腺素E2(PGE2)]水平以及治疗前、治疗6周、3个月牙周状况[探诊深度(PD)、龈沟出血指数(SBI)]。结果研究组总有效率97.14%(68/70),较对照组80.00%(56/70)高(P<0.05);治疗6周、3个月研究组PD浅于对照组,SBI少于对照组(P<0.05);治疗3 d,研究组NO、PGE2、IL-1β水平低于对照组(P<0.05);与对照组88.57%(62/70)对比,研究组美观满意度98.57%(69/70)明显较高(P<0.05)。结论牙冠延长术后桩核修复应用于牙体缺损至龈下残根残冠患者中,可增强治疗效果,改善牙周恢复状况,减轻龈沟液中炎性因子,并提高美观度。

Objective To observe the effect of post-core restoration after crown lengthening in patients with dental defect to subgingival residual root and residual crown.Methods A total of 140 patients with dental defect to subgingival residual root and residual crown received in our hospital from October 2016 to November 2019 were selected.According to the random number table method,they were divided into study group(70 cases)and control group(70 cases).The control group received direct post-core crown repair treatment.The study group received post-core restoration treatment after crown lengthening.The clinical efficacy,aesthetic satisfaction,the levels of inflammatory factors in gingival crevicular fluid[nitric oxide(NO),interleukin-1β(IL-1β)and prostaglandin E2(PGE2)]before and after treatment,periodontal status[probing depth(PD),sulcus bleeding index(SBI)]before treatment,6 weeks and 3 months of treatment were compared between the two groups.Results The total effective rate of the study group was 97.14%(68/70),which was higher than that of the control group 80.00%(56/70)(P<0.05).After 6 weeks and 3 months of treatment,the PD of the study group was lighter than that of the control group,and the SBI was less than that of the control group(P<0.05).After 3 days of treatment,the levels of NO,PGE2 and IL-1βin the study group were lower than those in the control group(P<0.05).Compared with 88.57%(62/70)of the control group,98.57%(69/70)of the study group's aesthetic satisfaction was significantly higher(P<0.05).Conclusion Post-core restoration after crown lengthening can be used in patients with dental defect to subgingival residual root and residual crown,which can enhance the treatment effect,improve the condition of periodontal recovery,reduce the inflammatory factors in the gingival crevicular fluid,and improve the aesthetics.
