比较经产妇与初产妇妊娠中晚期宫颈弹性参数 被引量:2

Comparison on cervical elasticity parameters in the second and third trimester of pregnancy between multiparas and primiparas

摘要 目的对比经产妇与初产妇妊娠中晚期宫颈超声弹性参数差异。方法选取96名产妇,其中初产妇48名(初产组)、经产妇48名(经产组),追踪妊娠结局均为足月自然分娩。采用经阴道超声宫颈弹性成像获取宫颈长度(CL)、闭合段宫颈内口应变率(CIS)、宫颈外口应变率(ES)、CIS/ES、超声弹性对比指数(ECI)及宫颈硬组织弹性占比(CHR),比较组间各指标差异。结果初产组孕晚期CL较孕中期缩短(P<0.05),CIS和CIS/ES升高(P均<0.05);经产组孕晚期CHR较孕中期降低(P<0.05);孕晚期经产期CIS和CIS/ES均较初产组降低(P均<0.05)。结论经产妇与初产妇孕期宫颈组织超声弹性参数存在一定差异,CIS、CIS/ES、CL和CHR伴随孕周增长具有不同变化特点。 Objective To compare cervical ultrasound elasticity indexes between multiparas and primiparas in the second and third trimester.Methods A total of 96 pregnant women were enrolled,including 48 primiparas(primiparas group)and 48 multiparas(multiparas group),the pregnancy outcomes were all natural delivery at term.Cervical length(CL),closed internal cervical os strain rate(CIS),external cervical os strain rate(ES),CIS/ES,elasticity contrast index(ECI)and cervical hardness ratio(CHR)were obtained with transvaginal ultrasound cervical tissue elasticity imaging,and indicators were compared between groups.Results In primiparas group,CL in the third trimester was shorter,while CIS and CIS/ES were higher than those in the second trimester(both P<0.05).In multiparas group,CHR in the third trimester was lower than that in the second trimester(P<0.05).In the third trimester,CIS and CIS/ES in multiparas group were lower than those in primiparas group(both P<0.05).Conclusion There were significant differences of ultrasound elasticity parameters of cervical tissue between multiparas and primiparas during pregnancy.With the increase of gestational age,CIS,CIS/ES,CL and CHR indicated more different characteristics between multiparas and primiparas.
