896例内分泌科非重症住院糖尿病患者床旁血糖数据分析

Analysis of point-of-care bedside glucose data of 896 non-critically ill patients with diabetes

目的分析内分泌科住院DM患者血糖管理情况。方法选取2018年1~10月于中南大学湘雅医院内分泌科住院治疗的非重症DM患者896例,采用Population、Patient、Patient-day 3种模型分析患者床旁快速血糖数据。结果共收集到43106个血糖数据,平均血糖为(9.60±4.30)mmol/L(Population模型)、(8.71±2.13)mmol/L(Patient模型)及(9.34±2.59)mmol/L(Patient-day模型),严格、一般及宽松控制率的总和为80%以上。高血糖、严重高血糖、低血糖和严重低血糖发生率Population模型为11.4%、4.9%,0.9%和0.1%,Patient模型为22.6%、57.4%、23.7%和5.2%,Patient-day模型为8.0%、16.8%、4.3%及0.6%。结论 3种模型平均血糖和血糖达标率基本类似,Patient-day模型可校正住院日和血糖监测次数的影响,适用于评价院内血糖管理的不良事件。

Objective To evaluate the inpatient glycemic control by point-of-care bedside glucose(POC-BG) data of non-critically ill patients with diabetes.Methods 896 patients with non severe DM who were hospitalized in the Department of Endocrinology, Xiangya Hospital of Central South University from January to October 2018 were selected.The bedside fast blood glucose data of patients were analyzed by three models:population, patient and patient day.Results A total of 43106 blood glucose data were analyzed.The mean blood glucose is(9.60±4.30) mmol/L(Population model),(8.71±2.13) mmol/L(Patient model),and(9.34±2.59) mmol/L(Patient-day model);the sum of strict,general and lose control rate is More than 80%;the incidence of hyperglycemia, severe hyperglycemia, hypoglycemia and severe hypoglycemia in the population model were 11.4%,4.9%,0.9% and 0.1%,respectively;the incidence of hyperglycemia severe hyperglycemia, hypoglycemia and severe hypoglycemia in the Patient model were22.6%,57.4%, 23.7% and 5.2% respectively;the incidence of hyperglycemia, severe hyperglycemia,hypoglycemia and severe hypoglycemia in the Patient-day model were 8.0%,16.8%,4.3% and 0.6%,respectively.Conclusion The mean blood glucose and the percent in different range of the three models are similar.The Patient-day model can correct the influence of hospitalization days and blood glucose monitoring frequencies and is suitable for evaluating adverse events of blood glucose management in the hospital.
