摘要
目的:观察布地奈德联合持续气道正压通气和肺表面活性物质治疗新生儿呼吸窘迫综合征患儿的效果。方法:选取80例新生儿呼吸窘迫综合征患儿作为研究对象,按随机数字表法将其分为对照组和研究组各40例。对照组采用持续气道正压通气联合肺表面活性物质治疗,研究组在对照组的基础上增加布地奈德治疗,比较两组治疗前后血气指标水平、呼吸机通气时间、住院时间、临床疗效和并发症发生率。结果:治疗后,研究组pH值和氧分压高于对照组,二氧化碳分压低于对照组,差异均有统计学意义(P<0.05);研究组呼吸机通气时间和住院时间短于对照组,差异均有统计学意义(P<0.05);研究组治疗总有效率为97.50%(39/40),明显高于对照组的80.00%(32/40),差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:布地奈德联合持续气道正压通气和肺表面活性物质治疗新生儿呼吸窘迫综合征患儿可提高治疗总有效率,改善血气指标水平,以及缩短呼吸机通气时间和住院时间,其效果优于持续气道正压通气联合肺表面活性物质治疗。
Objective:To study effects of Budesonide combined with continuous positive airway pressure ventilation and pulmonary surfactant in treatment neonatal respiratory distress syndrome.Methods:80 cases with neonatal respiratory distress syndrome were selected as the research objects,and were divided into control group and study group according to the random number table method,40 cases in each group.The control group was treated with continuous positive airway pressure ventilation combined with pulmonary surfactant treatment,while the study group was given Budesonide treatment on the basis of those of the control group.The blood gas index levels,ventilator ventilation time,hospitalization time,clinical efficacy and complication rate were compared between the two groups before and after the treatment.Results:After the treatment,the pH level and partial pressure of oxygen of the study group were higher than those of the control group;the partial pressure of carbon dioxide level was lower than that of the control group;and the differences were statistically significant(P<0.05).The ventilator ventilation time and hospitalization time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment in the study group was 97.50%(39/40),which was significantly higher than the control group of 80.00%(32/40),and the difference was statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:The triple treatment of Budesonide,continuous positive airway pressure ventilation and pulmonary surfactant in the treatment of neonatal respiratory distress syndrome can increase the total effective rate of treatment,improve the blood gas index levels,and shorten the ventilator ventilation time and hospitalization time.Moreover,it is superior to continuous positive airway pressure ventilation combined with pulmonary surfactant treatment.
