摘要
目的:观察纤支镜肺泡灌洗在老年肺部感染合并呼吸衰竭行有创机械通气患者中的应用效果。方法:选取100例老年肺部感染合并呼吸衰竭行有创机械通气患者作为研究对象,按随机数字表法将其分为对照组和研究组各50例。对照组予以常规吸痰治疗,研究组在对照组基础上联合纤支镜肺泡灌洗治疗,比较两组治疗前后相关疾病指标水平、住院时间、机械通气时间和血气指标水平。结果:治疗后,研究组血清降钙素原水平和超敏C反应蛋白水平均低于对照组,差异有统计学意义(P<0.05);研究组住院时间和有创通气时间均短于对照组,差异有统计学意义(P<0.05);研究组动脉血氧分压高于对照组,动脉血二氧化碳分压水平低于对照组,差异有统计学意义(P<0.05)。结论:在常规吸痰治疗基础上采用纤支镜肺泡灌洗可提高在老年肺部感染合并呼吸衰竭行有创机械通气患者动脉血氧分压水平,降低血清降钙素原水平、超敏C反应蛋白水平和动脉血二氧化碳分压水平,以及缩短住院时间和有创通气时间,效果优于单纯常规吸痰治疗。
Objective:To explore application effects of fiberoptic bronchoscopy alveolar lavage in invasive mechanical ventilation of elderly patients with lung infection and respiratory failure.Methods:100 elderly patients with lung infection and respiratory failure,who underwent invasive mechanical ventilation,were selected as the research objects.According to the random number table method,they were divided into control group and study group,each with 50 cases.The control group was given conventional sputum suction treatment,while the study group was combined with fiberoptic bronchoscopy alveolar lavage based on that of the control group.The relative disease index levels,hospitalization time,mechanical ventilation time and blood gas index levels before and after the treatment were compared between the two groups.Results:After the treatment,the levels of serum procalcitonin and high-sensitivity C-reactive protein in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The hospitalization time and invasive ventilation time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Further,the arterial partial pressure of oxygen in the study group was higher than that of the control group,and the arterial partial pressure of carbon dioxide was lower than the control group,and the differences were statistically significant(P<0.05).Conclusions:On the basis of conventional sputum suction treatment,the fiberoptic bronchoscopy alveolar lavage can increase the level of arterial oxygen partial pressure in the elderly patients with respiratory failure,and reduce the levels of serum procalcitonin,high-sensitivity C-reactive protein and arterial carbon dioxide partial pressure,and shorten the hospitalization time and invasive ventilation time.Moreover,it is superior to single conventional sputum suction treatment.
