四川地区膝骨关节炎患者生命质量调查及影响因素分析

Investigation on the Quality of Life of Knee Osteoarthritis Patients in Sichuan Area and Its Influencing Factors

目的探讨四川地区膝骨关节炎(KOA)患者生命质量现状及其影响因素,为提高KOA患者生命质量提供参考。方法运用横断面调查方法,采用一般资料调查表、西安大略和麦克马斯特大学骨关节炎指数(WOMAC指数)、《SF-12生命质量调查量表》对四川地区的237名KOA患者进行问卷调查,对不同特征KOA患者生命质量得分情况进行单因素分析,并建立多因素逐步回归模型分析KOA患者生命质量的影响因素。结果本研究四川地区KOA患者生理健康总评(PCS)得分为(42.20±6.12)分,心理健康总评(MCS)得分为(48.18±5.57)分,SF-12量表PCS和MCS维度得分均低于成都常模,差异有统计学意义(P<0.05)。多因素分析显示影响KOA患者PCS得分的影响因素有Kellgren&Lawrence影像学分级、病程及身体质量指数(BMI),影响MCS得分的因素为病程。WOMAC指数各维度得分与PCS得分之间存在负向关联。结论四川地区KOA患者总体生命质量较低,特别是生理健康方面,应针对影响因素采取针对性措施,改善KOA患者的生命质量。

Objective To investigate the status of life quality of knee osteoarthritis(KOA) patients in Sichuan area and its influencing factors,in order to provide references for improving the quality of life of KOA patients.Methods A cross-sectional survey method was used to conduct a questionnaire survey on 237 Sichuan KOA patients with the General Information Questionnaire,Western Ontario and McMaster University Osteoarthritis Index(WOMAC Index),and "The 12-item Short from Health Survey".The single-factor analysis was performed on the quality of life scores of KOA patients with different characteristics,and a multivariate stepwise regression model was built to analyze the factors affecting the quality of life of KOA patients.Results The physical component summary(PCS) scores of KOA patients in Sichuan area in this study was(42.20 ± 6.12)points,and the mental component summary(MCS) scores was(48.18±5.57) points.The scoress of the PCS and MCS dimensions of the SF-12 scale were lower than those of the Chengdu norm,and the differences were statistically significant(P<0.05).Multivariate analysis showed that the factors influencing the PCS scores of KOA patients were Kellgren & Lawrence imaging grading,course of disease and body mass index(BMI),and the factor that affected MCS scores was course of disease.There was a negative correlation between the scores of each dimension of the WOMAC index and the PCS scores.Conclusion Considering the overall quality of life of KOA patients in Sichuan area is relatively low,especially in the physical component summary,more targeted measures should be taken to improve the quality of life of KOA patients.
