Wiltse-open-TLIF手术治疗退变性腰椎失稳症的临床效果

Clinical Effect of Wiltse-open-TLIF Operation in the Treatment of Degenerative Lumbar Instability

目的比较Wiltse-open-TLIF手术与传统TLIF手术治疗退变性腰椎失稳患者的临床疗效。方法方便选取2016年3月—2019年9月万州区中医院脊柱外科所收治的126例退变性腰椎失稳患者,分为治疗组(Wiltse-open-TLIF)和对照组(TLIF),各63例,对两组患者临床疗效进行比较。结果两组患者术后下肢疼痛VAS评分,ODI评分、远期疗效比较差异无统计学意义(P>0.05);Wiltse-open-TLIF组与TLIF组术中出血量[(98.34±7.48)mL、(165.6±10.57)mL]、术后引流量[(48.12±6.53)mL、(127.32±23.64)mL]、术后卧床时间[(4.32±1.75)、(11.18±3.26)d],住院时间[(8.57±2.65)、(12.32±2.57)d],比较差异有统计学意义(t=4.890,2.760,5.280,4.630.P<0.05)。结论TLIF与Wiltse-open-TLIF对退变性腰椎失稳症的治疗效果均确切,但Wiltse-open-TLIF具有术中组织损伤小、术中出血少、术后引流少、恢复快等优势。

Objective To compare the clinical efficacy of Wiltse-open-TLIF surgery and traditional TLIF surgery in the treatment of patients with degenerative lumbar instability.Methods From March 2016 to September 2019,convenient select 126 patients with degenerative lumbar instability admitted to the Department of Spine Surgery,Wanzhou District Hospital of Traditional Chinese Medicine were divided into treatment group(Wiltse-open-TLIF)and control group(TLIF),with 63 cases in each group,compare the clinical efficacy of the two groups of patients.Results The VAS score,ODI score,and long-term efficacy of lower limb pain were not statistically significant between the two groups of patients after surgery(P>0.05);the intraoperative blood loss in the Wiltse-open-TLIF group and the TLIF group was[(98.34±7.48)mL,(165.6±10.57)mL],postoperative drainage volume[(48.12±6.53)mL,(127.32±23.64)mL],postoperative bed time[(4.32±1.75)d,(11.18±3.26)d],hospital stay[(8.57±2.65)d,(12.32±2.57)d],the difference was statistically significant(t=4.890,2.760,5.280,4.630,P<0.05).Conclusion Both TLIF and Wiltse-open-TLIF are effective in the treatment of degenerative lumbar spine instability,but Wiltse-open-TLIF has the advantages of less intraoperative tissue damage,less intraoperative bleeding,less postoperative drainage,and faster recovery.
