慢性心力衰竭患者BNP、cTnI、Hcy表达与室性心律失常相关性分析

Correlation among levels of BNP,cTnI,and Hcy expression and ventricular arrhythmia in patients with chronic heart failure

目的:探讨慢性心力衰竭(CHF)患者脑钠肽(BNP)、肌钙蛋白I(cTnI)、同型半胱氨酸(Hcy)表达水平与室性心律失常(VA)的相关性。方法:选取2016年7月至2019年6月云浮市人民医院收治的60例CHF伴VA患者作为观察组,另选取42例同期体检健康者作为对照组。根据Lown法对VA患者分级,比较各组BNP、cTnI、Hcy水平差异,并对BNP、cTnI、Hcy的表达与VA的相关性进行分析。结果:观察组BNP、cTnI、Hcy水平分别为[(428.74±132.19)pg/mL、(0.51±0.14)ng/mL、(23.62±5.41)μmol/L],显著高于对照组的[(58.36±12.62)pg/mL、(0.05±0.01)ng/mL、(9.24±2.89)μmol/L](P<0.01);随着VA分级的增加,CHF患者BNP、cTnI水平呈上升趋势,其中Ⅳ~Ⅴ级BNP、cTnI水平高于Ⅱ~Ⅲ级及Ⅰ级(P<0.05)。观察组BNP与cTnI、Hcy水平均呈正相关(r s=0.512,r s=0.724,P<0.05),cTnI与Hcy水平呈正相关(r s=0.427,P<0.05);观察组BNP、Hcy水平与VA无显著相关(P>0.05),cTnI与VA呈中等程度相关(r s=0.412,P<0.05)。结论:CHF伴VA患者BNP、cTnI、Hcy水平高于正常,随Lown分级增加BNP、cTnI呈上升趋势;cTnI与VA呈正相关。

Objective:To investigate the correlation among the expression of brain natriuretic peptide(BNP),cardiac troponin I(cTnI)and homocysteine(Hcy),and ventricular arrhythmia(VA)in patients with chronic heart failure(CHF).Methods:Sixty CHF patients with VA admitted to Yunfu People’s Hospital between July 2016 and June 2019 were enrolled as the study group,and a contemporary cohort of 42 healthy check-up subjects were selected as the control group.The VA in the patients were graded according to Lown classification.The levels of BNP,cTnI,and Hcy were compared among the study group and the control group,as well as among patients with different Lown classifications.The correlation among the expression levles of BNP,cTnI,and Hcy,and VA,was analyzed.Results:The levels of BNP,cTnI and Hcy in the study group were(428.74±132.19)pg/mL,(0.51±0.14)ng/mL,and(23.62±5.41)μmol/L,which were significantly higher than those in the control group[(58.36)±12.62)pg/mL,(0.05±0.01)ng/mL,(9.24±2.89)μmol/L,respectively](P<0.01).With higher VA class,the levels of BNP and cTnI in the CHF patients showed an upward trend.The levels of BNP and cTnI were higher in patients with classes IV-V VA than those with classes Ⅱ-Ⅲ or class I VA(P<0.05).In the study group,BNP was positively correlated with cTnI and Hcy levels(r s=0.512,r s=0.724,P<0.05);cTnI was positively correlated with Hcy level(r s=0.427,P<0.05).In the study group,the BNP and Hcy did not significantly correlated(P>0.05),whereas the cTnI was moderately correlated(r s=0.412,P<0.05),with VA.Conclusion:In CHF patients with VA,the levels of BNP,cTnI,and Hcy are higher than normal;BNP and cTnI tend to increase with the higher Lown classification;cTnI is positively correlated with VA.
