地中海贫血铁过载脏器的MRI影像学特征分析及其与MRI T2*值的关系分析 被引量:2

摘要 目的:探讨地中海贫血铁过载脏器的MRI影像学特征分析及其与MRI T2*值的关系。方法:回顾性选取90例重型β地中海贫血患者作为研究对象,所有患者均进行了心脏、肝脏、胰腺、肾脏器官的MRI检查,同时测定了血清铁蛋白(SF),收集其临床资料,分析各器官MRI T2*值之间及与SF间的关系。结果:90例患者SF平均值为(2596.13±982.36)μg/L。根据SF进行铁过载程度的分组,三组间肝脏与胰腺T2*值存在统计学差异,均随着SF过载程度的加重而降低(P<0.05)。pearson相关性分析结果显示,SF与肝脏及胰腺T2*值呈显著负相关(P<0.05),与心脏、肾脏以及脾脏的T2*值均无相关性(P>0.05)。肝脏和脾脏、肝脏和胰腺间的T2*值呈显著正相关(P<0.05),其他器官间的T2*值均无相关性(P>0.05)结论:重型β地中海贫血患者容易发生心脏、肝脏等器官铁过载,过载脏器MRI T2*值呈现减低,其中血清铁负荷与肝脏、胰腺T2*值存在显著负相关。 Objective:To investigate the imaging characteristics of iron-overloaded organs and its relationship with T2** measurement on MRI in thalassemia.Methods:Ninety patients withβ-thalassemia major were included retrospectively.All patients underwent MRI examinations of the heart,liver,pancreas,and kidney;meanwhile,serum ferritin(SF)was measured,and clinical data were collected.The relationship between MRI T2* measurement and SF in these organs was analyzed.Results:The mean SF level was(2596.13±982.36)μg/L in the 90 patients.The subjects were divided into three groups according to severity of iron overloading classified by the SF levels.The T2* measurements of the liver and pancreas statistically differed across the three groups,and all decreased with the increase of SF overload(P<0.05).Pearson correlation analysis showed that SF was significantly negatively correlated with T2* values of the liver and pancreas(P<0.05),but was not correlated with those of the heart,kidney,and spleen(P>0.05).There were significantly positive correlations in T2* values between the liver and spleen,and between the liver and pancreas(P<0.05),but not among other organs(P>0.05).Conclusion:Patients withβ-thalassemia major are prone to iron overload in organs such as the heart and liver.The MRI T2* meaurement is lower in iron-overloaded organs.There are significant negative correlations between serum iron load and the T2* measurements in the liver and pancreas.
